经皮穿刺椎体成形术对老年骨质疏松椎体压缩性骨折患者术后骨水泥渗漏率的影响

Effect of Percutaneous Vertebroplasty on Bone Cement Leakage Rate in Elderly Patients with Osteoporotic Vertebral Compression Fractures

目的:探讨经皮穿刺椎体成形术对老年骨质疏松椎体压缩性骨折患者术后骨水泥渗漏率的影响。方法:选择2016年7月-2018年7月在我院就诊的骨质疏松性压缩性骨折患者168例作为研究对象,采用随机数字表法将患者分为PKP组和PVP组各84例。PVP组给予经皮椎体成形术治疗,PKP组给予经皮椎体后凸成形术治疗。观察2组手术前后视觉模拟疼痛评分(VAS)、伤椎高度变化,统计2组骨水泥渗漏率。结果:2组术后VAS与治疗前相比较均显著降低(P<0.05),但术后2组VAS相比较差异无统计学意义(P>0.05);2组术后伤椎高度与术前相比较显著升高(P<0.05),且PVP组伤椎高度高于对照组(P<0.05)。PVP组骨水泥渗漏率显著高于PKP组(P<0.05)。结论:与PKP相比较,PVP治疗老年骨质疏松性压缩性骨折具有较高的骨水泥渗漏率,但PKP存在术后伤椎高度丢失,临床可根据实际情况选择适合的手术方式。

Objective:To investigate the effect of percutaneous vertebroplasty on bone cement leakage rate in elderly patients with osteoporotic vertebral compression fractures.Methods:From July 2016 to July 2018,168 patients with osteoporotic compression fractures were selected as subjects.The patients were divided into PKP group and PVP group by random number table method,84 cases in each group.The group PVP was treated with percutaneous vertebroplasty and group PKP was treated with percutaneous kyphoplasty.Visual analogue pain score(VAS)and vertebral height were observed before and after operation in the two groups.Bone cement leakage rate was calculated in the two groups.Results:The postoperative VAS in both groups was significantly lower than that before treatment(P<0.05),but there was no significant difference between the two groups(P>0.05).The height of injured vertebrae in the 2 groups increased significantly compared with that before operation(P<0.05),and the height of injured vertebrae in group PVP was higher than that in control group(P<0.05).The leakage rate of PVP cement was significantly higher than that of PKP group(P<0.05=..Conclusion:Compared with PKP,PVP treatment of osteoporosis compression fractures in the elderly has a high leakage rate of bone cement,but PKP has a loss of postoperative spinal height,and clinic can choose a suitable surgical method according to the actual situation.
