三种常见胃大部切除术断端吻合方式对患者术后恢复影响的比较

Comparison of the Effects of Three Common Anastomotic Methods on the Recovery of Patients after Subtotal Gastrectomy

目的 比较BillrothⅠ式、BillrothⅡ式和Roux-en-Y式对胃大部切除术患者术后恢复的影响。方法 收集2015年1月-2018年12月于盘锦市中医医院进行胃大部切除术的患者147例,分为BillrothⅠ式组、BillrothⅡ式组和Roux-en-Y式组,各49例。比较各组患者术后肠道菌群变化情况和肠道功能恢复情况。结果Roux-en-Y式组各菌落的变化值均为最小,与其他两组比较差异有统计学意义(P<0.05)。Roux-en-Y式组肠道恢复情况明显优于BillrothⅠ式组和BillrothⅡ式组,Visick分级差异有统计学意义(P<0.05)。结论 采用Roux-en-Y式吻合作为胃大部切除术后的吻合方式,对肠道菌群干扰小,肠道功能恢复快,可作为首选吻合方式。

Objective To compare the effects of Billroth I, Billroth II and Roux-en-Y anastomosis on the recovery of patients after subtotal gastrectomy. Methods A total of 147 patients who underwent subtotal gastrectomy in our hospital from January 2015 to December 2018 were divided into Billroth I group, Billroth II group and Roux-en-Y group (49 cases in each group). The changes of intestinal flora and the recovery of intestinal function were compared among the groups. Results Compared with Billroth I group and Billroth II group, the changes of each colony in Roux-en-Y group were the smallest (P<0.05). The recovery of intestinal tract in Roux-en-Y group was significantly better than that Billroth I group and Billroth II group (P<0.05). Conclusion Roux-en-Y anastomosis is the first choice for anastomosis after subtotal gastrectomy, which has less interference to intestinal flora and faster recovery of intestinal function.
