留置鼻胆管患者非计划性拔管的影响因素分析

Influencing factors for unplanned extubation of indwelling nasobiliary drainage tube

目的探讨留置鼻胆管患者非计划性拔管可能的影响因素,为制定防止非计划性拔管策略提供依据。方法采用便利抽样法选取2018年1—10月在杭州市第一人民医院消化内科行治疗性内镜逆行胰胆管造影术(ERCP)术后留置鼻胆管的患者1 682例为研究对象。回顾性收集其临床资料及非计划性拔管发生情况,对非计划性拔管影响因素进行单因素分析和二分类非条件Logistic回归分析。结果1 682例留置鼻胆管患者中,住院期间发生非计划性拔管44例,发生率为2.62%,Logistic回归分析发现,年龄、性别、文化程度、感觉障碍和留置时间是患者非计划性拔管的影响因素。结论留置鼻胆管患者发生非计划性拔管是多种因素综合作用的结果,临床护理中应采取综合有效措施预防非计划性拔管的发生。

Objective To explore the possible influencing factors of unplanned extubation in patients with indwelling nasobiliary tube,and to provide basis for formulating strategies to prevent unplanned extubation.Methods A total of 1 682 patients with indwelling nasobiliary tube after undergoing endoscopic retrograde cholangiopancreatography(ERCP)in Department of Gastroenterology of Hangzhou First People's Hospital from January to October 2018 were selected by convenience sampling method.The clinical data and the occurrence of unplanned extubation were retrospectively collected.The influencing factors of unplanned extubation were analyzed by univariate analysis and binary unconditional Logistic regression analysis.Results Among 1 682 patients with indwelling nasobiliary tube,44 patients had unplanned extubation during hospitalization,the incidence was 2.62%.Logistic regression analysis showed that age,sex,education level,sensory disturbance and indwelling time were the influencing factors of unplanned extubation.Conclusions The unplanned extubation in patients with indwelling nasobiliary tube is the result of multiple factors.Comprehensive and effective measures should be taken to prevent unplanned extubation in clinical nursing.
