摘要
目的:探讨血清25-羟维生素D325-(OH)D3联合前白蛋白(Prealbumin,PA)在儿童社区获得性肺炎(Community-acquiredPneumonia,CAP)严重度评估中的应用价值。方法:选取2018年1月~2019年2月我院收治符合标准的123例CAP患者,根据2007美国感染病学会/美国胸科学会共同发布的社区获得性肺炎诊治标准将其分为轻症肺炎组(n=41)和重症肺炎组(n=82)。采用随机数字法收集选取同期本院保健科进行健康体检的98例健康儿童作为对照组。测定三组儿童的血清25-(OH)D3、PA,观察其在三组的血清PA和血清25-(OH)D3变化。结果:重症肺炎组和轻症肺炎组血清PA分别为(91.36±6.23)mg/L、(128.67±20.34)mg/L,均低于对照组(289.62±24.63)mg/LP<0.05),重症肺炎血清PA低于轻症肺炎组P<0.05)。重症肺炎组和轻症肺炎组的血清血清25-(OH)D3分别为(20.56±7.32)ng/mL、(26.35±10.64)ng/mL,均于对照组的(37.69±10.27)ng/mLP<0.05),重症肺炎组血清25-(OH)D3低于轻症肺炎组P<0.05)结论:血清25-(OH)D3和PA与CAP炎密切相关,而且其与患儿病情程度呈现负相关,检测血清25-(OH)D3和PA对CAP的早期评估具有一定意义。
Objective:To investigate the application value of serum 25-hydroxyvitamin D3(25-(OH)D3)combined with prealbumin(PA)in the assessment of severity of community-acquired pneumonia(CAP)in children. Methods:123 CAP patients admitted to our hospital from January 2018 to February 2019 who meet the criteria were selected and divided into mild pneumonia group(n=41)and severe pneumonia group(n=82)according to the 2007 American Society of Infectious Diseases/American Thoracic Society Jointly Issued Diagnosis and Treatment Criteria for Community Acquired Pneumonia. The random number method was used to collect and select 98 healthy children who underwent physical examination in the department of health care of our hospital during the same period as the control group. Serum 25-(OH)D3 and PA were measured in three groups.Results:The serum PA levels in severe pneumonia group and mild pneumonia group were higher than those in the control group((91.36±6.23 )mg/L vs(128.67±20.34)mg/L vs(289.62±24.63) mg/L,[KG-*8] P<0.05),and the serum PA levels in severe pneumonia group were lower than that in mild pneumonia group([KG-*8] P<0.05). Severe pneumonia group and mild pneumonia group of PA in the control group(P<0.05),severe pneumonia group serum 25-(OH)D3 is lower than the mild pneumonia group((20.56±7.32)ng/mLvs(26.35±10.64)ng/mL vs(37.69±10.27)ng/mL,P<0.05).Conclusion:The serum 25-(OH)D3 and PA is closely related to the CAP inflammation,and with the children negatively associating with the degree of illness,detection of serum 25-(OH)D3 and PA is significant to the early evaluation of CAP.
