初产妇第二产程中应用自由体位分娩的临床效果观察

Clinical effect of free position delivery in the second stage of labor of primipara

目的 探讨初产妇第二产程中应用自由体位分娩的临床效果。方法 本研究选取2018年3月-2018年9月期间于我院分娩的200例初产妇为研究对象,采用随机数字表法将其分为2组,第一产程2组均采取常规护理措施,第二产程对照组采用传统膀胱截石位、观察组采用自由体位分娩,比较不同体位的临床效果。结果 相较于对照组产妇,观察组第二产程耗时缩短、产后2h出血量降低(P<0.05);观察组受试产妇会阴侧切率显著低于对照组(P<0.05),2组间新生儿窒息率差异无统计学意义(P>0.05)。结论 初产妇第二产程中应用自由体位分娩可缩短第二产程耗时、降低产后出血量及会阴侧切率,且不增加新生儿窒息率。

Objective To investigate the clinical effect of free position delivery in the second stage of labor of primipara. Methods In this study, 200 primipara delivered in our hospital from March 2018 to September 2018 were selected as the research objects.They were divided into two groups by random number method. Routine nursing measures were adopted in both groups during the first stage of labor.The control group used traditional supine position or lithotomy position during the second stage of labor, and the observation group used free body as delivery, compared the clinical effects of different positions. Results Compared with the control group, the second stage of labor in the observation group was shorter, and the amount of postpartum bleeding in 2 hours was lower (P<0.05). The lateral perineal incision rate in the observation group was significantly lower than that in the control group (P<0.05), and there was no significant difference in neonatal asphyxia rate between the two groups (P>0.05). Conclusion The application of free position delivery in the second stage of labor of primipara can shorten the second stage of labor, reduce the amount of postpartum hemorrhage and the rate of lateral perineal incision, and do not increase the rate of neonatal asphyxia.
