麦味五参汤加减治疗COPD稳定期肺肾两虚型患者的临床疗效及对肺功能的影响

Effect of Maiwei Wushen Decoction on Stable COPD with Deficiency of both Lung and Kidney and its Influence on BODE index,Immune Function and Pulmonary Function

目的探讨麦味五参汤加减治疗慢性阻塞性肺疾病(COPD)稳定期肺肾两虚型患者的疗效及对BODE指数、免疫功能和肺功能的影响。方法选取2016年1月-2018年6月于我院就诊的96例COPD稳定期肺肾两虚型患者作为研究对象,按照随机数字表法分为两组,对照组患者给予噻托溴铵粉吸入剂治疗,试验组在此基础上服用麦味五参汤,均用药3个月。观察两组患者用药后的临床疗效,免疫因子CD4^+、CD8^+水平,肺功能FVC、FEV1指标及用药前后的BODE指数。结果用药前两组患者BODE指数无显著性差异,治疗后均降低,但试验组变化幅度更明显,两者有显著性差异(P<0.05),用药后,试验组患者的CD4^+、FVC、FEV1均显著高于对照组,CD8^+显著低于对照组,两者有显著性差异(P<0.05),试验组95.83%患者的COPD症状得到有效改善,总有效率高于对照组中的79.17%,两者有显著性差异(P<0.05)。结论麦味五参汤不仅能够促进COPD稳定期肺肾两虚型患者的肺功能恢复、改善呼吸困难,还可缓解气流阻塞、提高免疫力。

Objective To investigate the effect of Maiwei Wushen Decoction on stable COPD of deficiency of both lung and kidney and its influence on BODE index,immune function and pulmonary function.Methods 96 patients with stable COPD of lung and kidney deficiency who were admitted to our hospital from January 2016 to June 2018 were enrolled in the study.They were divided into two groups according to the random number table method.The control group was given tiotropium bromide powder inhalation treatment while the experimental group,on this basis,took Maiwei Wushen Decoction,all for 3 months.The clinical efficacy of the two groups of patients after treatment,the levels of immune factors CD4^+,CD8^+,lung function FVC,FEV1 and BODE index before and after treatment were observed.Results There was no significant difference in BODE index between the two groups before treatment,but the index decreased in both groups after treatment,and the change of the experimental group was more obvious.There was significant difference between the two groups (P<0.05).After treatment,the CD4^+,FVC,FEV1 of the experimental group was significantly higher than the control group,CD8^+ was significantly lower than the control group,and there was a significant difference between the two ( P <0.05).95.83% of the test group had effective improvement of COPD symptoms,and the total effective rate was higher than that of the control group (79.17%).There was a significant difference between the two ( P <0.05).Conclusion Maiwei Wushen Decoction can not only promote the recovery of lung function and improve dyspnea in patients with stable lung and kidney deficiency,but also relieve airflow obstruction and improve immunity.
