医护一体化床旁交接单在泌尿外科病房患者交接中的应用

目的探讨医护一体化床旁交接单在泌尿外科病房患者交接中的应用效果。方法自行设计一种泌尿外科专用的医护一体化床旁交接单,将患者的基本信息、疾病信息、医护一体化查房信息、未完成信息、其他信息记录在床旁交接单上,且每天在前1d的基础上累加记录,确保患者从入院到出院整个过程的重点内容均体现在床旁交接单上。结果实行床旁交接单交接班后,护士对患者病情掌握程度以及医生对护士满意度较实施前明显提高,交接班问题发生率较实施前明显降低,差异有统计学意义(P<0.05)。结论 使用专科化的床旁交接单实现无缝交接,有利于各班护士快速、全面并动态地掌握患者病情,减少交接班问题发生率,并强化医护间的信息沟通与合作。对提高护理质量,降低医疗风险,提高医生对护士满意度有较好的临床意义。

Objective To explore the development and application of the medical integrated bedside delivery list in the urological ward. Methods A special integrated bedside delivery list for urology was self designed,which recorded the basic information,disease information,integrated medical and nursing information,unfinished information and other information on the bedside delivery list,and were accumulated on the basis of the previous day,so as to ensure the key contents of the whole process from admission to exit hospital were reflected in the bedside delivery list. Results After the implementation of bedside delivery list,the nurses mastery of patients condition and the doctors satisfaction with nurses were significantly higher than before,and the incidence of handover problems was significantly lower,with statistically significant ( P <0.05). Conclusions Using specialized bedside delivery list to achieve seamless handover was beneficial to grasp of the patient s conditionquickly,comprehensively and dynamically,reduce the incidence of handover problems,and strengthen the information communication and cooperation between the the medical care.It is good to increase the quality of nursing,reduce medical risks and improve doctors satisfaction with nurses.
