摘要 目的探讨微小RNA-149(MiR-149)rs22928323位点基因多态性与类风湿关节炎(RA)发病的关系。方法选取确诊的RA患者90例(RA组)、健康自愿者90例(健康组),收集时间2014年1月-2016年12月,采用聚合酶链反应(PCR)技术检测两组外周血MiR-149 rs22928323位点的基因型、等位基因频率,并采用多因素分析法研究MiR-149 rs22928323位点基因多态性与RA发病的关系。结果 RA组患者的MiR-149 rs22928323位点TC基因型50.00%高于健康组TC基因型的28.89%(P<0.05),RA组的等位基因T的频率51.67%显著的高于健康组的等位基因T频率36.67%(P<0.05);RA组和健康组的年龄、性别、白细胞(WBC)、血小板(PLT)、尿WBC比较,差异均无统计学意义(P>0.05);RA组的尿蛋白、抗角蛋白抗体(Anti-keratin antibody ,AKA)阳性率、抗环瓜氨酸肽抗体(Anti-cyclic citrullinated peptide antibody ,CCP)阳性率均显著的高于健康组(P<0.05);Logistic回归分析显示:AKA阳性表达、CCP阳性表达、等位基因T的频率增加是人群RA发病的独立危险因素(P<0.05)。结论 RNA-149(MiR-149)rs22928323位点等位基因T频率增加是人群RA发病的独立危险因素。 Objective To investigate the relationship between the polymorphism of MiR-149 rs22928323 locus and the pathogenesis of RA. Methods Ninety patients with RA diagnosed (RA group) and 90 healthy volunteers (healthy group) collected from January 2014 to December 2016 were enrolled. The genotype and allele frequency of the peripheral blood MiR-149 rs22928323 locus were detected by PCR. The relationship between the polymorphism of MiR-149 rs22928323 locus and the pathogenesis of RA was studied by multivariate analysis. Results In the RA group, the TC genotype of MiR-149 rs22928323 was 50.00% higher than that of the healthy group TC genotype 28.89%(P<0.05). The frequency of allele T in the RA group was 51.67% significantly higher than that in the healthy group 36.67%(P<0.05);There were no significant differences in age, gender, WBC, PLT and urinary WBC between RA group and healthy group (P>0.05). Urinary protein, AKA positive rate and CCP positive rate in RA group were significantly higher than those in healthy group (P<0.05);Logistic regression analysis showed that AKA positive expression, CCP positive expression, and increased frequency of allele T were independent risk factors for RA (P<0.05).Conclusion Increased T-frequency of the RNA-149 (MiR-149) rs22928323 locus allele is an independent risk factor for the onset of RA in the population.
