单枚cage融合单侧钉棒固定治疗腰椎间盘突出合并腰椎不稳症 被引量:2

摘要 目的探讨单侧开窗减压髓核摘除单枚cage融合单侧钉棒固定治疗腰椎间盘突出合并腰椎不稳症的疗效。方法 对68例腰椎间盘突出合并腰椎不稳症患者采用单侧开窗减压髓核摘除单枚cage融合单侧钉棒固定治疗。结果 患者均获得随访,近期随访时间1~3年,中期随访时间5~10年。根据MacNab疗效评定标准评定疗效,近期随访优39例,良25例,可4例,优良率94.1%;中期随访优32例,良29例,可7例,优良率89.7%;两时段的疗效比较差异无统计学意义(P>0.05)。结论 单侧开窗减压髓核摘除单枚cage融合单侧钉棒固定治疗腰椎间盘突出合并腰椎不稳症可获得良好的临床疗效。 Objective To investigate the efficacy of unilateral fenestration decompression discectomy and unilateral screw-rod system fixation with single cage fusion for the treatment of lumbar disc herniation combined with lumbar spinal instability. Methods The 68 patients diagnosed as lumbar disc herniation combined with lumbar spinal instability were performed unilateral fenestration decompression discectomy and unilateral screw-rod system fixation with single cage fusion. Results All patients were followed up, the time was 1~3 years for short term,and 5~10 years for medium term. According to MacNab four-grade scale,the short-term follow-up results were excellent in 39 cases,good 25 and fair 4.The excellent and good rate was 94.1%.The median-term follow-up results were excellent in 32 cases,good 29 and fair 7. The excellent and good rate was 89.7%. No significant difference was observed between median-term and short-term efficacy( P >0.05). Conclusions Unilateral fenestration decompression discectomy and unilateral screw-rod system fixation with single cage fusion for lumbar disc herniation combined with lumbar spinal instability can obtain good efficacy.
