锁定钢板与外固定架治疗肱骨干骨不连的疗效

The efficacy of humeral shaft nonunion treated with locking plate and external fixator

目的比较锁定钢板与外固定架治疗肱骨干骨不连的疗效。方法 将63例肱骨干骨折术后骨不连患者随机分为锁定钢板组(31例)和外固定架组(32例),观察两组患者的手术情况及治疗效果等。结果 患者均获得随访,时间10~12(11.3±1.4)个月。两组手术时间、出血量、切口长度、术后引流量、并发症发生率比较差异均无统计学意义(P>0.05)。骨折愈合时间、治疗有效率锁定钢板组优于外固定架组(P<0.05)。末次随访时,锁定钢板组的肩肘关节功能恢复情况优于外固定架组(P<0.01)。结论 锁定钢板内固定联合自体松质骨植骨治疗肱骨干骨折术后骨不连有利于骨折愈合,肩、肘关节功能恢复快,效果显著。

Objective To compare the efficacy of locking plate and external fixator in the treatment of nonunion of humeral shaft. Methods The 63 patients with nonunion after humeral shaft fracture were randomly divided into the locking plate group (31 cases) and the external fixation group (32 cases). The operation condition and therapeutic effects of the two groups were observed. Results All patients were followed up for 10~12 (11.3±1.4) months. There were no significant differences in the operation time, blood loss, length of incision, postoperative drainage and complication rate between the two groups ( P >0.05). The time of fracture healing and the effective rate of treatment of the locking plate group were better than those of the external fixation group ( P <0.05). At the last follow-up, the functional recovery of the shoulder and elbow joints of the locking plate group were better than those of the external fixation group ( P <0.01). Conclusions Locking plate internal fixation combined with autologous cancellous bone grafting are used in the treatment of nonunion after humeral shaft fracture, which can be beneficial to fracture healing and rapid recovery of shoulder and elbow joint function,with significant effect.
