空心钉联合内侧支撑钢板治疗青壮年Pawuells Ⅲ型股骨颈骨折

Treatment of Pawuells type Ⅲ femoral neck fracture in young adults with cannulated screw and medial supporting plate

目的探讨在传统空心钉内固定基础上加用内侧支撑钢板治疗青壮年PawuellsⅢ型股骨颈骨折的效果。方法 对21例PawuellsⅢ型股骨颈骨折患者在闭合复位失败后行切开复位、空心钉内固定联合股骨颈内侧支撑钢板固定。结果 21例患者中1例死亡,1例失访,19例获得随访,时间6~12个月。骨折全部愈合,时间6~14周。术后10周按Harris评分标准评价髋关节功能:优良15例,较好3例,尚可1例。结论 青壮年PawuellsⅢ型股骨颈骨折应用空心钉及内侧支撑钢板固定效果肯定。

Objective To investigate the therapeutic efficacy of the Pawuells type Ⅲ femoral neck fracture in young adults treated with the traditional cannulated screw associated with medial supporting plate. Methods The 21 cases of the Pawuells type Ⅲ femoral neck fracture were selected, and firstly adopted closed reduction and internal fixation, when reduction failed, switching to open reduction and internal fixation,which was fixed by the cannulated screw associated with medial supporting plate of femoral neck. Results In 21 cases, 1 case was lost to follow-up, 1 case died and the rest 19 patients were followed up for 6~12 months, who all acquired bone union. The fracture healing time was 6~14 weeks. At 10 weeks postoperation,according to Harris score of hip function: 15 cases were excellent,3 good, 1 fair. Conclusions Treatment of the Pawuells type Ⅲ femoral neck fracture in young adults with the cannulated screw associated with medial supporting plate are dramatically effective.
