2018年某教学医院ICU医院感染目标性监测研究报告 被引量:6

Targeted monitoring on nosocomial infections in ICU of a teaching hospital in 2018

摘要 目的了解海南某教学医院综合ICU病房医院感染的现况,为制定针对性的干预措施提供参考。方法对2018年1-12月入住该院综合ICU的患者进行前瞻性目标性监测,并统计分析医院感染、3种导管相关感染及医院感染病原菌分布情况。结果共监测患者488例,发生医院感染46例,医院感染发病率为9.43%,医院感染例次数61例,医院感染例次率为12.5%。各季度医院感染例次率比较有统计学差异(χ^2=11.467,P=0.009)。医院感染部位以下呼吸道感染为主(54.10%)。共培养出病原菌共429株,其中革兰阴性菌272株,占63.40%,检出病原菌第一位的菌株为鲍曼不动杆菌。结论开展目标性监测能及时发现感染控制工作中的薄弱环节,有利于制定有效的防控与干预措施,降低ICU医院感染发病率。 Objective To understand the current status of nosocomial infections(also called healthcare-associated infection,HAI)in patients in an ICU(Intensive Care Unit)of a teaching hospital,so as to provide a reference for constituting targeted intervention measures.Methods prospective objective monitoring method was used to monitor the status of HIA in patients who admitted to this ICU from January to December 2018.The condition of three types of catheter-related infections and the distribution of pathogens were also be analyzed statistically in this study.Results A total of 488 patients were surveyed,46 patients developed 61 times HIA,HIA rate and HIA case rate were 9.43% and 12.5% respectively.Incidence of HIA case between the four seasons was significantly different(χ^2=11.467,P=0.009).The main infection site was lower respiratory tract(54.10%).A total of 429 pathogenic isolates were detected,63.40%(n=272)of which were gram-negative bacteria.Among all pathogens,the top one was Acinetobacter baumannii.Conclusion Objective monitoring can timely reflect the weak point in infection control practice,is beneficial to formulating effective prevention and intervention measures to reduce the incidence of nosocomial infections in ICU.
