心血管内科患者PCI术后出现感染的高危因素分析

Analysis of Risk Factors for Infection in Patients with Cardiovascular Disease after PCI

目的分析心血管内科患者冠状动脉介入(PCI)术后出现感染的高危因素。方法回顾性分析2014年1月-2018年12月2在我院心血管内科行PCI手术且术后发生感染的23例患者的临床资料,收集患者性别、年龄、BMI、慢性疾病史、长期吸烟史、术前发热情况、意识障碍、手术时间、胸腔引流时间、呼吸机使用时间、导尿管留置时间、探视次数、住院时间,分析其出现感染高危因素及病原菌分布。结果年龄、高血压病史、术前发热、探视次数、手术时间、呼吸机使用时间、导尿管留置时间、住院时间是术后感染独立危险因素,有统计学差异(P<0.05);感染以下呼吸道为主,主要致病菌是革兰阴性菌。结论术后感染高危因素多,应重视术前感染因素并做好术中、术后护理,降低PCI术后感染率。

Objective To investigate risk factors for infection in patients with cardiovascular disease after percutaneous coronary intervention(PCI).Methods Collecting Between January 2014 and December 2018,the clinical data of 23 patients with post-operative infection after PCI in cardiovascular department of our hospital were retrospectively analyzed.Data including gender,age,BMI,chronic disease history,long-term smoking history,preoperative fever,consciousness disorder,operation time,thoracic drainage time,ventilator use time,indwelling time of catheter,visiting times and hospitalization time were collected.Then the infection risk factors and pathogenic bacteria distribution of the infection were determined.Results Age,history of hypertension,preoperative fever,visitation times,operation time,ventilator use time,indwelling time of catheter and hospitalization time were independent risk factors for post-operative infection(P<0.05).The lower respiratory tract was the main infection site,and the main pathogenic bacteria were Gram-negative bacteria.Conclusion There are many high risk factors of postoperative infection,so preoperative infection factors,intraoperative and postoperative nursing should be monitoredto reduce the infection rate after PCI.
