甲状腺全切除术加中央组淋巴结清扫术在分化型甲状腺癌临床治疗中的应用

Application of total thyroidectomy plus central lymph node dissection in the clinical treatment of differentiated thyroid cancer

目的探讨甲状腺全切除术加中央组淋巴结清扫术在分化型甲状腺癌临床治疗中的应用效果。方法选择我院60例分化型甲状腺癌患者(2013年1月-2015年12月),随机分为2组各30例,两组均行甲状腺全切除术,观察组术中行中央组淋巴结清扫术,而对照组未对中央组淋巴结予以清扫,比较手术情况、近期疗效、局部复发率、中远期存活率。结果两组的手术时间、术中出血量、客观缓解率比较均无统计学差异(P>0.05),但随访1年、3年,观察组的局部复发率较对照组更低(P<0.05),其存活率较对照组更高(P<0.05)。结论中央组淋巴结清扫术可减少分化型甲状腺癌患者行甲状腺全切除术后的局部复发,有利于提高患者中远期存活率。

Objective To investigate the clinical effect of total thyroidectomy plus central lymphadenectomy in the treatment of differentiated thyroid cancer.Methods 60 patients with differentiated thyroid cancer in our hospital(January 2013 to December 2015)were randomly divided into two groups,30 cases in each group.Total thyroidectomy was performed in both groups,central group lymph node dissection was performed in observation group,while central group lymph node dissection was not performed in control group.The operation condition,short-term effect,local recurrence rate and long-term survival rate were compared.Results There was no significant difference in operation time,bleeding volume and objective remission rate between the two groups(P>0.05),but after 1 or 3 years of follow-up,the local recurrence rate of the observation group was lower than the control group(P<0.05),and the survival rate of the observation group was higher than the control group(P<0.05).Conclusion Central lymphadenectomy can reduce local recurrence of differentiated thyroid cancer patients after total thyroidectomy,and improve the long-term survival rate of patients.
