摘要
目的观察丁苯酞注射液联合强化步行训练对早期缺血性脑卒中偏瘫患者下肢功能恢复的效果。方法选取医院收治的脑卒中偏瘫患者72例,按入院顺序随机分为试验组和对照组,各36例。两组均进行早期联合强化步行训练,试验组在此基础上予以丁苯酞注射液治疗,治疗2周。结果治疗后,两组患者神经功能缺损(NIHSS)评分均较治疗前降低(P <0. 05),巴氏(Barthel)指数均较治疗前升高(P <0. 05),且上述指标变化幅度均大于对照组(P <0. 05);试验组患者治疗总有效率为94. 44%,明显高于对照组的63. 89%(P <0. 05);两组患者治疗2周后血清神经损伤标志物[S100β蛋白(S100β)、同型半胱氨酸(HCY)]水平均较治疗前降低(P <0. 05),且试验组降幅更大(P <0. 05);两组患者治疗6周后的Fugl-Meyer评定量表(FMA)、Holden功能性步行分级法(FAC)评分均明显高于治疗前(P <0. 05),且试验组变化幅度较大(P <0. 05)。结论早期脑卒中偏瘫患者予以丁苯酞联合强化步行训练,可更有效地减轻神经功能损伤,促进下肢运动功能恢复。
Objective To observe the effect of Butylphthalide Injection combined with intensive walking training on the recovery of lower limb function in patients with early ischemic stroke and hemiplegia. Methods Totally 72 patients with stroke and hemiplegia admitted to our hospital were selected and randomly divided into the experimental group and the control group according to the order of admission,36 cases in each group. The patients in the two groups were given early combined intensive walking training,on this basis,the patients in the experimental group were treated with Butylphthalide Injection. Both groups were treated for two weeks. Results After treatment,the National Institutes of Health Stroke Scale( NIHSS) score in the two groups was lower than that before treatment( P < 0. 05),while the Barthel index in the two groups was higher than that before treatment( P < 0. 05),and the change of the above indexes in the experimental group was more significant than that in the control group( P < 0. 05). The total effective rate of the experimental group was 94. 44%,which was significantly higher than 63. 89% of the control group( P < 0. 05). After two weeks of treatment,the levels of serum nerve injury markers [ S100β protein( S100β) and homocysteine( HCY)] in the two groups were lower than those before treatment( P < 0. 05),and the decrease in the experimental group was more significant( P < 0. 05). After six weeks of treatment,the Fugl-Meyer Rating Scale( FMA) and Holden Functional Ambulation Category( FAC) score in the two groups were significantly higher than those before treatment( P < 0. 05),and the change in the experimental group was more significant( P < 0. 05). Conclusion Butylphthalide combined with intensive walking training can effectively alleviate neurological impairment and promote the recovery of lower limb motor function in patients with early stroke and hemiplegia.
