70 kVp结合SAFIRE在头颈部CTA中的应用价值

Application of 70 kVp combined with SAFIRE in CTA of head and neck

目的探讨70kVp结合正弦图确定迭代重建(Sinogram Affirmed Iterative Reconstruction,SAFIRE)技术在头颈部CT血管造影术(CTA)中的应用价值。方法选取2018年1月至2018年10月于大连市第三人民医院放射科行头颈部CTA扫描患者60例,其中6月至10月患者30例(男17例,女13例)、年龄42~76岁,70kVp结合SAFIRE,对比剂用量:0.5mL/kg,为实验组;1月至5月患者30例(男16例,女14例),年龄34~75岁,120kVp结合滤波反投影重建(Filtered Back Projection,FBP),对比剂用量:80mL,为对照组。对比两组的对比剂用量,CT剂量指数(CTDIvol),CT值,对比噪声比(CNR)及图像质量评分。结果实验组对比剂用量为(41.12±8.13)mL,对照组为80mL,两组具有统计学差异(P<0.01);实验组CTDIvol为(8.31±3.63)mGy,对照组为(13.41±4.02)mGy,两组具有统计学差异(P<0.01);实验组双侧颈总动脉、双侧颈外动脉、双侧颈内动脉、双侧大脑中动脉、双侧椎动脉V2段及基底动脉的CT值和CNR均优于对照组(P均<0.05);实验组的图像质量评分为(4.21±0.31),对照组为(4.04±1.01),两组差异无统计学意义(P>0.05)。结论70kVp结合SAFIRE技术在头颈部CTA扫描中,即可以降低辐射剂量、减少碘对比剂的用量,又改善了头颈部CTA的图像质量,适于临床推广。

Objective To explore the application value of 70 kVp combined with SAFIRE(Sinogram Affirmed Iterative Reconstruction) in CTA of head and neck.Methods Totally 60 patients,who underwent head and neck CTA in the Department of Radiology at the Third People's Hospital of Dalian from January 2018 to October 2018,were divided into experimental group and control group.The experimental group included 30 cases from June to October (male 17,female 13;age range 42-76 years old).The experimental group was given 0.5 mL/kg contrast agent with 70 kVp and SAFIRE.The control group included 30 cases from January to May(male 16,female 14;age range 34-75 years old).The control group was given 80 mL contrast agent with 120 kVp and FBP(Filtered Back Projection,FBP).Contrast dosage,CTDIvol,Image quality,CT value and CNR of the two groups were compared.Results The dosages of contrast agent were (41.12±8.13)mL in experimental group and 80 mL in control group ( P< 0.01).CTDlvol was (8.31±3.63)mGy in experimental group and(13.41±4.02)mGy in control group ( P< 0.01).The CT value and CNR of double CCA、ECA、ICA、MCA、VA and BA in experimental group were better than those in control group ( P> 0.05).The image quality scores were 4.21±0.31 in experimental group and 4.04±1.01 in control group ( P> 0.05).Conclusion 70 kVp combined with SAFIRE technology can reduce the radiation dose and dosage of iodine contrast agent,and improve the image quality of head and neck CTA,thus is worthwhile to recommended for clinical application.
