闭合复位交锁髓内钉固定治疗股骨干骨折的临床研究

Clinical Study of Closed Reduction and Interlocking Intramedullary Nailing in the Treatment of Femoral Shaft Fractures

目的:观察闭合复位交锁髓内钉固定治疗股骨干骨折的临床效果。方法:选取2013年3月-2018年2月笔者所在医院骨科收治的90例股骨干骨折患者作为本次研究对象,按随机数表法分为研究组(n=45)与对照组(n=45)。对照组采用常规切开复位术治疗,研究组采用闭合复位交锁髓内钉固定治疗,比较两组手术疗效、手术时间、术中出血量、术后骨折愈合时间及并发症发生情况,并随访1年,对比两组患者生活质量变化情况。结果:研究组手术优良率88.9%,较对照组的71.1%明显更高(P<0.05);研究组手术时间、术后骨折愈合时间较对照组明显更短(P<0.05),术中出血量、并发症发生率较对照组更低(P<0.05);术后1年时,研究组患者各项生活质量评分均显著高于术前和对照组(P<0.05)。结论:闭合复位交锁髓内钉固定治疗股骨干骨折疗效确切,手术时间短、术中出血量少,可促进患者术后骨折愈合,且并发症较少,有助于提高患者的生活质量,临床推广和应用价值高。

Objective:To observe the clinical effect of closed reduction and interlocking intramedullary nailing in the treatment of femoral shaft fractures.Method:Ninety patients with femoral shaft fractures admitted to our department from March 2013 to February 2018 were enrolled in this study.They were divided into study group(n=45) and control group(n=45) by random number table.The control group was treated with conventional open reduction and the study group was treated with closed reduction and interlocking intramedullary nailing.The surgical outcome,operation time,intraoperative blood loss,postoperative fracture healing time and complications were compared and followed up 1 year,the quality of life of the two groups was compared.Result:The excellent and good operation rate of the study group was 88.9%,which was significantly higher than that of the control group(P<0.05).The operation time and postoperative fracture healing time of the study group were significantly shorter than those of the control group(P<0.05).The intraoperative blood loss and complication rate were lower than the control group(P<0.05).At 1 year postoperatively,the quality of life scores of the study group were significantly higher than those of the preoperative and control groups(P<0.05).Conclusion:Closed reduction and interlocking intramedullary nailing is effective in the treatment of femoral shaft fractures.Short operation time and less intraoperative blood loss can promote postoperative fracture healing,and fewer complications,which can improve the quality of life of patients.Clinical promotion and application value is high.
