胃充盈超声造影与内镜检查对胃食管反流病的诊断价值

Diagnosis value of gastroesophageal reflux disease by gastric-filling ultrasound and endoscope

目的探讨胃充盈超声造影与内镜检查对胃食管反流病(Gastroesophageal Reflux Disease,GERD)的诊断价值。方法收集2011年3月~2018年9月临床最终诊断的64例GERD数据,分析胃食管充盈超声造影和内镜检查的结果,比较两种检测方法对GERD的检出率及其临床诊断价值。结果胃充盈超声造影检出52例,阳性率81.3%(52/64),未检出12例,阴性率18.7%(12/64);内镜检出31例,阳性率48.4%(31/64),未检出33例,阴性率51.6%(33/64);胃充盈超声造影检出率高于内镜检查(P<0.05)。结论胃充盈超声造影检查可操作性强、可实时观察有无反流声像,较内镜诊断GERD直观、简便、无痛苦、无不良反应、检出率高,可作为临床诊断GERD的首诊检查方法。

Objective To study the diagnosis value gastroesophageal reflux disease(GRED) by gastric-filling ultrasound and endoscope. Methods 64 GERD patients in clinical final diagnosis were collected from March 2011 to September 2018. A retrospective analysis was conducted by comparing the results of gastroesophageal filling ultrasound with endoscope and summarized the characteristics of ultrasound images of GERD. The detection rate of the two methods and clinical diagnostic value were explored. Results In 64 GERD cases, 52 cases were detected by gastric filling ultrasound and positive rate was 81%(52/64). 12 cases were not detected and negative rate was 18%(12/64). 31 cases were detected by endoscope and positive rate was 48%(31/64). 33 cases were not detected. The negative rate was 51%(33/64). The difference of detection rate in two ways was statistically significant. Conclusion Gastric filling ultrasound examination has strong operability with real-time observation of reflux vocal image. Compare with endoscopic diagnosis of GERD, gastric filling ultrasound is intuitive, simple, painless, no adverse reaction and high detection rate.
