血清CRP及尿DPd的变化及与种植体周围炎的相关性 被引量:5

Changes of Serum CRP and Urinary DPd in Patients with Dental Implants and Their Correlation with Peri-implantitis

摘要 为了探讨口腔种植修复患者血清CRP及尿DPd的变化及与种植体周围炎的相关性,本研究选择本院收治的人工种植修复患者100例作为研究对象,根据是否发生种植体周围炎分为种植修复组(n=83)和种植体周围炎组(n=17)。选择同期健康体检者作为对照组(n=30)。分别监测不同时间段的各组受试者的血清C反应蛋白(CRP)及尿脱氧吡啶啉(DPd)水平。研究结果表明:种植修复组患者在术后1周时血清CRP水平显著升高(p<0.05),然后在术后1个月呈现下降趋势,术后3个月时的血清CRP水平与术前无显著差异(p>0.05)。种植修复组患者的尿DPd水平在术后1周后显著升高(p<0.05),并且在术后1个月达到最高,此后至3个月时一直保持较高水平。种植体周围炎组患者治疗前的血清CRP和尿DPd水平均显著高于健康对照组(p<0.05)。治疗1周后,患者血清CRP水平较治疗前显著下降。然而,种植体周围炎患者的尿DPd水平在整个治疗过程中始终保持较高水平(p>0.05)。种植体周围炎患者的血清CRP和尿DPd水平在甲硝唑治疗前显著高于未感染者和健康体检者(p<0.05)。两组患者的血清CRP和尿DPd水平呈显著正相关(p<0.05)。初步的研究结论表明:种植体周围炎可导致口腔种植修复患者的血清CRP和尿DPd浓度显著升高,并且CRP和DPd参与牙槽骨的吸收转化。 To investigate the changes of serum C-reactive protein (CRP) and urinary deoxypyridinoline (DPd) in patients with dental implants and their correlation with peri-implantitis. 100 patients who underwent artificial implant restoration in our hospital were selected from January 2016 to June 2017 as subjects. According to whether or not peri-implantitis occurred, patients were divided into the implant-repair group (n=83) and peri-implantitis group (n=17). The same period of healthy physical examination was selected as the control group (n=30). Serum CRP and urinary DPd levels were monitored for each group of subjects at different time periods. The results showed that serum CRP levels were significantly increased in the implant-repair group at 1 week postoperatively (p<0.05), and then decreased at 1 month postoperatively. Serum CRP levels at 3 months postoperatively were not significantly different from those before surgery (p>0.05). The level of urinary DPd in the implant-repair group was significantly increased after 1 week (p<0.05), and reached the highest at 1 month after surgery, and remained high until 3 months thereafter. Serum CRP and urinary DPd levels were significantly higher in the peri-implantation group than in the healthy controls (p<0.05). After 1 week of treatment, the serum CRP level of the patients decreased significantly compared with that before treatment. However, urinary DPd levels in patients with peri-implantitis remained high throughout the course of treatment (p>0.05). Serum CRP and urinary DPd levels in patients with peri-implantitis before treatment with metronidazole were significantly higher than those in uninfected and healthy subjects (p<0.05). In the two groups, serum CRP and urinary DPd levels were significantly positively correlated with each other (p<0.05). Preliminary findings suggested that peri-implantitis can lead to a significant increase in serum CRP and urinary DPd levels in patients with oral implants, CRP and DPd are involved in the absorption and transformation of alveolar bone.
