后腹腔镜右侧肾癌根治术技术改进的临床研究

Modified retroperitoneal laparoscopic radical nephrectomy for right renal cell carcinoma

目的探讨后腹腔镜右侧肾癌根治性切除术技术改进的手术效果。方法对潍坊市人民医院2015年9月至2018年9月收治的52例右侧肾癌患者的临床资料进行回顾性分析和总结,将52例患者随机分为治疗组(n=28)和对照组(n=24),治疗组行后腹腔镜肾癌根治性改进术,对照组行传统后腹腔镜手术治疗,比较两组术中出血量、手术时间及术后恢复情况。结果治疗组手术时间、术中出血量、术后住院时间均少于对照组(P<0.05);术后肠功能恢复时间与对照组比较,差异未见统计学意义(P>0.05)。两组术中及术后未出现明显并发症,随访1年未见肿瘤复发及穿刺通道的种植性转移。结论与传统腹腔镜根治性肾癌切除术相比,改进后的后腹腔镜肾癌根治术具有手术时间短、出血少、住院天数少等优点,值得临床推广应用。

Objective To investigate the effects of modified retroperitoneal laparoscopic radical nephrectomy on right renal cell carcinoma. Methods The clinical data of 52 patients with right renal cell carcinoma admitted to Weifang People’s Hospital from September 2015 to September 2018 were retrospectively analyzed and summarized. The 52 patients were randomly divided into treatment group (n=28) and control group (n=24). The treatment group received modified retroperitoneal laparoscopic nephrectomy, while the control group received traditional retroperitoneal laparoscopic surgery. The intraoperative bleeding volume, operation time and postoperative recovery were compared between the two groups. Results The operation time, intraoperative bleeding volume and hospitalization time of the treatment group were lower than those of the control group (P<0.05), and the recovery time of intestinal function after operation was shorter than that of the control group, but the difference was not significant (P>0.05). There were no obvious complications during and after operation in the two groups. And no recurrence of tumors and implantation metastasis of puncture passage were found in the follow-up period of 1 year. Conclusions Compared with the traditional laparoscopic radical nephrectomy, the modified retroperitoneal laparoscopic radical nephrectomy has the advantages of shorter operation time, less bleeding and shorter hospitalization, which is worthy of clinical application.
