白血病血小板无效输注的影响因素分析

Influencing factors of platelet ineffective transfusion for leukemia patients

目的研究白血病血小板无效输注(PTR)的影响因素。方法回顾性分析2016年12月至2018年12月濮阳市人民医院行血小板输注治疗的白血病患者62例的临床资料,共完成血小板输注174次,根据是否发生PTR分为PTR组和有效组,比较两组临床资料并采用多因素Logistics回归分析PTR发生影响因素。结果62例患者共完成血小板输注174次,其中有效输注118次,PTR发生56次,PTR发生率为32.18%;单因素分析显示PTR发生可能与发热、感染、脾功能亢进、抗生素应用、活动性出血及血小板输注次数有关(P<0.05);Logistics回归分析显示,PTR发生的独立危险因素包括发热、感染、脾功能亢进、活动性出血及血小板输注次数(P<0.05)。结论白血病血小板输注治疗PTR发生风险较高,其独立危险因素包括发热、感染、脾功能亢进、活动性出血及血小板输注次数。

Objective To study the influencing factors of the efficacy of platelet-ineffective transfusion (PTR) leukemia. Methods Clinical data of 62 leukemia patients treated by platelet transfusion from December 2016 to December 2018 in Puyang People’s Hospital were retrospectively analyzed, a total of 174 platelet transfusions were completed. The patients were divided into PTR group and effective group according to whether PTR occurred. The clinical data of the two groups were compared, and multivariate iogistic regression analysis was used to analyze the factors affecting PTR. Results A total of 174 platelet transfusions were completed in 62 patients, including 118 effective infusions and 56 PTR events. The incidence of PTR was 32.18%. Univariate analysis showed that PTR may be associated with fever, infection, hypersplenism, antibiotic use, active bleeding, and platelet transfusion times (P<0.05). Logistic regression analysis showed that independent risk factors for PTR included fever, infection, hypersplenism, active bleeding, and platelet transfusion times (P<0.05). Conclusions Leukemia platelet transfusion has a higher risk of PTR. The independent risk factors include fever, infection, hypersplenism, active bleeding, and platelet transfusion times.
