摘要
目的探讨胃癌患者胃切除后消化道重建术中应用P型空肠袢代胃术治疗对其生存质量的影响。方法抽取巴彦淖尔市医院2017年1月至2018年10月诊治的74例胃癌患者,按随机数字表法分为观察组和对照组,每组37例。两组患者均接受全胃切除术,观察组患者术后采用P型空肠袢代胃术治疗,对照组患者采用常规Lahey+Braun吻合术治疗。观察两组患者的围术期指标及术后并发症发生情况。结果观察组手术用时、住院时间均短于对照组,术中出血量少于对照组,术后进食量、进食次数以及体质量增加情况均优于对照组,差异均有统计学意义(P均<0.05);观察组术后并发症发生率为5.41%,低于同期对照组的24.32%(P<0.05)。两组患者治疗前各项生活质量评分均较低,差异未见统计学意义(P>0.05);治疗后观察组各项生活质量指标评分均高于对照组(P均<0.05)。结论针对胃癌患者胃切除后消化道重建术中应用P型空肠袢代胃术有助于优化围术期指标,降低术后并发症发生风险,可显著改善患者术后的生存质量。
Objective To analyze the effects of P-type jejunal sputum gastrostomy on the quality of life of patients with gastric cancer after gastrectomy. Methods Clinical data from 74 patients with gastric cancer who were treated in Bayannur Hospital from January 2017 to October 2018 were retrospectively analyzed. Patients were grouped by random number table method, with 37 cases in each group. All of patients in the two groups underwent total gastrectomy. Patients in the observation group were treated by P-type jejunum after gastrostomy. While patients in the control group were treated by conventional Lahey+ Braun anastomosis. The perioperative indexes and complications of the two groups were observed. Results The operation time and hospital stay of the observation group were significantly shorter than those of the control group;the intraoperative blood loss was significantly less than that of the control group;the postoperative food intake and frequency, body weight were significantly higher than those of the control group;all the differences were statistically significant (P all< 0.05). The postoperative complication rate of the observation group was 5.41%, lower than the 24.32% of the control group (P<0.05). The postoperative scores of quality of life in the two groups were lower before treatment, and there was no significant difference between the two groups (P>0.05). After treatment, the quality of life scores of the observation group were significantly higher than those of the control group (P<0.05). Conclusions The application of P-type jejunal degeneration in the reconstruction of digestive tract after gastric resection in gastric cancer patients can help to optimize the perioperative index and reduce the risk of postoperative complications, which can significantly improve the quality of life after surgery.
