重组人血小板生成素治疗小儿特发性血小板减少性紫癜48例临床观察

Clinical observation of recombinant human thrombopoietin in the treatment of 48 children with idiopathic thrombocytopenic purpura

目的观察重组人血小板生成素(rhTPO)治疗小儿特发性血小板减少性紫癫(ITP)的临床价值。方法选取2016年1月至2018年12月48例ITP患儿为研究对象,所有患儿在常规对症支持治疗的基础上加用rhTPO治疗。观察患儿的治疗效果、外周血小板(PLT)计数及不良反应发生情况。结果 48例ITP患儿经治疗完全反应(CR)患儿18例,有效(R)患儿23例,无效(NR)患儿7例,治疗总有效率为85.42%(41/48)。患儿治疗后4周PLT计数较治疗前显著上升,差异有统计学意义(P<0.05);患儿不良反应率为8.33%(4/48)。结论在ITP患儿治疗中应用rhTPO治疗具有疗效显著和安全性高等优势,可有效控制患儿的出血症状,rhTPO值得在小儿ITP治疗中应用与推广。

Objective To observe the clinical value of recombinant human thrombopoietin(rhTPO) on children with idiopathic thrombocytopenic purpura(ITP). Methods Forty-eight children with ITP from January 2016 to December 2018 were selected as the objects of study. All children were treated with rhTPO on the basis of routine symptomatic support therapy. The therapeutic effects, peripheral platelet(PLT) counts and adverse reactions of children were compared. Results Among the 48 children with ITP after treatment, 18 children with complete response(CR), 23 children with effective(R), and 7 children with ineffective(NR), the total effective rate of treatment was 85.42 %(41/48). After 4 weeks of treatment, the PLT count increased significantly than before treatment, and the difference was significant(P<0.05). The adverse reaction rate was 8.33 %(4/48). Conclusions The rhTPO treatment in the treatment of children with ITP has significant efficacy and high safety advantages. It can effectively control the symptoms of bleeding in children. rhTPO is worth applying and promoting in the treatment of children with ITP.
