基础阻抗对消融指数指导射频消融组织损伤范围影响 被引量:3

Effect of fundamental impedance on tissue damage range guided by ablation index

摘要 目的探讨在消融指数(AI)指导下,不同基础阻抗对射频消融组织损伤范围的影响。方法模拟人体内部环境,在猪心心外膜侧心肌组织进行射频消融。将猪心分为A、B两组,A组应用不同AI值行射频消融,观察不同AI时,损伤深度及宽度的变化特征;B调整基础阻抗行射频消融,测量不同基础阻抗时对损伤深度及宽度的影响。结果在基础阻抗相对恒定时,消融损伤深度、宽度与AI呈明显正相关(r=0.933、0.902,P<0.05)。AI值较为固定时,消融损伤深度、宽度与基础阻抗呈明显负相关(r=-0.930、-0.920,P<0.05)。结论将基础阻抗整合在内的新型AI值可增加消融范围预测精确度,为射频消融提供帮助。 Objective To investigate the effect of different fundamental impedances on the extent of tissue injury in radiofrequency ablation(RFA)under the guidance of ablation index(AI).Methods Radiofrequency ablation was performed in the myocardium on the epicardial side of the pig heart,simulating the internal human environment.Pig hearts were divided into two groups:Group A and Group B.In Group A,different AI values were applied for radiofrequency ablation to observe the variation characteristics of injury depth and width at different AI.In Group B,the basic impedance for radiofrequency ablation was adjusted,and the influence of different basic impedance on the damage depth and width was measured.Results When the basic impedance was relatively constant,the depth and width of ablation damage were positively correlated with AI(r=0.933,0.902,P<0.05).When AI value was relatively fixed,ablative damage depth and width were negatively correlated with basic impedance(r=-0.930,-0.920,P<0.05).Conclusion A new AI value incorporating fundamental impedance can increase the accuracy of ablation range prediction and provide assistance for radiofrequency ablation.
