高压氧治疗患者的焦虑及耳痛程度分析

Assessment of Earache and Anxiety in Patients Undergoing Hyperbaric Oxygen Therapy

目的调查高压氧治疗患者的精神状况和耳痛不适反应,分析其可能的影响因素,为选择干预性治疗提供临床依据。方法选取310例行高压氧治疗的患者,应用自行设计的调查表对其进行调查。采用状态焦虑特质问卷(STAI)、视觉模拟评分(VAS)分别对患者的焦虑和耳痛反应进行评估。结果高压氧治疗患者存在着不同程度的焦虑,焦虑程度与文化程度、婚姻、病程有关(P<0.01)。耳痛以轻、中度不适为主,尤以加压时为甚,未见重度耳痛;患者咽鼓管状况和身体状况为耳痛发生的独立因素(P<0.05);焦虑程度与耳痛呈正相关(r=0.4560~0.7140,P<0.0001)。结论高压氧治疗中患者焦虑和耳痛存在调压方式、年龄、文化程度等方面的差别,对于特殊患者应针对性实施干预。

Objective To investigate the mental status and earache in patients undergoing hyperbaric oxygen therapy and to analyze the possible influencing factors,so as to provide a clinical basis for the choice of interventional therapy. Methods A total of 310 patients who underwent hyperbaric oxygen therapy were surveyed by a self-designed questionnaire.The State-Trait Anxiety Inventory(STAI) and Visual Analogue Scale(VAS) were used to evaluate the anxiety and earache,respectively. Results Patients undergoing hyperbaric oxygen therapy had varying degrees of anxiety,and the degree of anxiety was related to education level,marital status and illness duration.Patients mainly exhibited mild-to-moderate earache,especially under pressurized condition( P < 0.05).No severe earache was found in all patients.The status of eustachian tube and physical condition were the independent factors for earache( P <0.05).The degree of anxiety was positively correlated with earache( r =0.456 0~0.714 0, P <0.000 1). Conclusion There are differences in pressure regulation,age and education level among patients with anxiety and earache in hyperbaric oxygen therapy.Therefore,interventions should be targeted to specific patients.
