腹腔镜下卵巢囊肿剥除术后创面电凝和缝合止血对残留卵巢储备功能的影响

Effect of wound electrocoagulation and suture hemostasis on residual ovarian reserve function after laparoscopic ovarian cyst removal

目的探讨腹腔镜下卵巢囊肿剥除术后创面电凝和缝合止血对残留卵巢储备功能的影响。方法选择2017年1月至2019年1月于青岛市第六人民医院就诊并行腹腔镜下卵巢囊肿剥除术治疗的患者124例进行前瞻性研究,采用随机数字表法将患者分为试验组与对照组,两组各62例。试验组患者术后采用缝合止血,对照组患者术后采用创面电凝止血,比较两组治疗前后的性激素指标、双侧卵巢储备卵泡数量及卵巢功能恢复情况。结果试验组患者术后3个月的促卵泡成熟激素(FSH)(7.92±2.76 pg/ml)、黄体生成激素(LH)(5.86±1.48 m U/ml)水平显著低于对照组的(9.95±2.30 pg/ml)、(7.10±2.04 m U/ml),雌二醇(E2)水平(125.78±30.44 m U/ml)显著高于对照组(119.25±27.91 m U/ml),两组比较差异具有统计学意义(P<0.05)。试验组患者术后1 d(6.27±2.33个)、3个月(8.21±2.89个)的双侧卵巢储备卵泡数量显著少于对照组(5.39±2.40个,7.35±1.77个),差异具有统计学意义(P<0.05)。试验组的月经不规律情况、围绝经期综合表现情况均明显优于对照组,两组比较差异具有统计学意义(P<0.05)。结论腹腔镜下卵巢囊肿剥除术后缝合止血对患者残留卵巢储备功能影响小,有利于术后卵巢功能恢复,效果良好,值得推荐。

Objective To investigate the effect of wound electrocoagulation and suture hemostasis on residual ovarian reserve function after laparoscopic ovarian cyst removal.Methods 124 cases of patients admitted to Qingdao sixth people's hospital from January 2017 to January 2019 for laparoscopic ovarian cyst removal were selected for prospective study.Patients were divided into experimental group and control group by random number table method,with 62 cases in each group.Patients in the experimental group were treated with suture for hemostasis,while patients in the control group were treated with wound electrocoagulation for hemostasis.The sex hormone indexes,the number of bilateral ovarian reserved follicles and the recovery of ovarian function before and after treatment were compared between the two groups.Results Follicular maturation hormone(FSH)(7.92±2.76 pg/ml),luteal progesterone(LH)(5.86±1.48 mU/ml),estradiol(E2)(125.78±30.44 mU/ml)were significantly lower than those of the control group(9.95±2.30 pg/ml),(7.10±2.04 mU/ml),and(125.78±30.44 mU/ml)than those of the control group(119.25±27.91 mU/ml).The differences between the two groups were statistically significant(P<0.05).The number of bilateral ovarian reserve follicles in the experimental group(6.27±2.33)and 3 months after operation(8.21±2.89)was significantly lower than that in the control group(5.39±2.40)and(7.35±1.77),with statistically significant differences(P<0.05).The comprehensive performance of menstrual recovery and perimenopausal period in the experimental group was significantly better than that in the control group,and the difference between the two groups was statistically significant(P<0.05).Conclusion Suture hemostasis after laparoscopic ovarian cyst removal has little effect on the residual ovarian reserve function of patients,which is beneficial to the recovery of ovarian function after surgery,and the effect is good,worthy of recommendation.
