大承气汤联合低分子肝素治疗急性胰腺炎的效果观察

Effects of Dachengqi Decoction Combined with Low Molecular Weight Heparin on Acute Pancreatitis

目的:观察大承气汤联合低分子肝素治疗急性胰腺炎的效果。方法:纳入2015年1月至2018年12月期间我院收治的78例急性胰腺炎患者为受试对象,采用随机数字表法分为实验组(大承气汤联合低分子肝素治疗组)、对照组(低分子肝素治疗组)各39例。比较两组患者治疗7d后临床疗效,治疗前、治疗7d后实验室指标[降钙素原(PCT)、C反应蛋白(CRP)、白细胞、血淀粉酶],以及不良反应发生情况。结果:治疗7d后,实验组患者治疗总有效率明显高于对照组(P<0.05);两组患者血清PCT、CRP、白细胞、血淀粉酶水平均较治疗前明显降低(P均<0.05),且实验组明显低于对照组(P均<0.05);两组不良反应发生率对比差异无统计学意义(P>0.05)。结论:大承气汤联合低分子肝素治疗急性胰腺炎可明显缓解炎症反应,改善症状,疗效显著且安全性高。

Objective:To observe the effects of Dachengqi decoction combined with low molecular weight heparin in the treatment of acute pancreatitis.Methods:A total of 78 patients with acute pancreatitis admitted to our hospital from January 2015 to December 2018 were enrolled in the study and were divided into experimental group(Dachengqi decoction combined with low molecular weight heparin treatment group)and control group(low molecular weight heparin treatment group)according to the random number table method,with 39 cases in each group.The clinical efficacy after 7d of treatment,laboratory indicators[procalcitonin(PCT),C-reactive protein(CRP),leukocytes,blood amylase]before treatment and after 7d of treatment and occurrence of adverse reactions were compared between the two groups.Results:After 7d of treatment,the total effective rate of treatment in experimental group was significantly higher than that in control group(P<0.05).The levels of serum PCT,CRP,leukocytes and blood amylase in the two groups were significantly decreased compared with those before treatment(all P<0.05),and the levels in experimental group were significantly lower than those in control group(all P<0.05).There was no significant difference in the incidence rate of adverse reactions between the two groups(P>0.05).Conclusion:Dachengqi decoction combined with low molecular weight heparin can significantly alleviate the inflammatory response and improve symptoms in the treatment of acute pancreatitis,and it has remarkable efficacy and high safety.
