乳腺癌术后患者夫妻亲密关系在心理韧性和创伤后成长之间的中介作用

The Mediating Effect of Marital Intimate Relationship between Psychological Resilience and Post-traumatic Growth for Patients after Breast Cancer Surgery

目的:探讨乳腺癌术后患者的心理韧性、夫妻亲密关系和创伤后成长之间关系,并进一步分析夫妻亲密关系在心理韧性和创伤后成长之间的中介作用,为临床干预提供新依据。方法:采用一般情况调查问卷、创伤后成长评定量表(Posttraumatic Growth Inventory,PTGI)、14个条目心理韧性量表(14 items Resilience Scale,RS-14)、亲密关系满意度问卷(Quality of Relationship Index,QRI)对160例乳腺癌术后患者进行匿名问卷调查。结果:本组乳腺癌患者的心理韧性总均分为(62.119±12.443)分,夫妻亲密关系总均分为(31.056±7.498)分,创伤后成长总均分为(59.500±10.487)分。Pearson相关性分析显示,乳腺癌患者心理韧性、亲密关系和创伤后成长之间两两均呈正相关(P<0.001)。中介效应检验显示,夫妻亲密关系在心理韧性和创伤后成长之间起部分中介作用,中介效应为22.5%,占总效应的35.71%。结论:夫妻亲密关系是乳腺癌患者心理韧性和创伤后成长的中介因素,心理韧性可以改善患者的夫妻亲密关系,从而促进其创伤后成长。

Objective:To explore the relationship among psychological resilience,marital intimacy and post-traumatic growth in patients with breast cancer after surgery,and further analyze the mediating effect of marital intimacy on psychological resilience and post-traumatic growth,so as to provide a new basis for clinical intervention.Methods:A total of 160 patients with post-traumatic Growth Inventory(PTGI),14 items Resilience Scale(RS-14)and Quality of Relationship Index(QRI)were surveyed anonymously.Results:The total score of psychological resilience of the patients with breast cancer was(62.119±12.443),the total score of intimate relationship between husband and wife was(31.056±7.498),and the total score of post-traumatic growth was(59.500±10.487).Pearson correlation analysis showed that there was a positive correlation between psychological resilience,intimacy and post-traumatic growth in breast cancer patients(P<0.001).Mediating effect test showed that the intimate relationship between husband and wife played a part of mediating effect between psychological resilience and post-traumatic growth,which was 22.5%,accounting for 35.71% of the total effect.Conclusion:The intimate relationship between husband and wife is an intermediary factor for the psychological resilience and post-traumatic growth of breast cancer patients.Psychological resilience can improve a patient′s marital intimacy relationship with her spouse,thereby facilitating post-traumatic growth.
