血清PCT和血乳酸在小儿肺炎的表达水平及与预后的关系 被引量:4

摘要 目的:探讨血清降钙素原(Procalcitonin,PCT)和血乳酸在小儿肺炎的表达水平,并分析其与小儿重症肺炎的关系。方法:选取我院小儿肺炎159例,根据是否为重症肺炎分为普通肺炎组(108例)和重症肺炎组(51例),同时选取同期来我院体检的健康小儿50例作为对照组,根据重症肺炎患儿是否死亡,又分为存活亚组40例和死亡亚组11例;记录各组患儿年龄、性别及发病时间,并检测各组患儿入院时血清PCT和血乳酸水平,比较各组PCT和血乳酸水平的差异,并分析其与重症肺炎预后的关系。结果:普通肺炎组与对照组血乳酸比较无统计学意义(P>0.05),但重症肺炎组与普通肺炎组、对照组比较,血乳酸差异有统计学意义(P<0.05);重症肺炎组及普通肺炎组血清PCT均显著高于对照组,比较差异均有统计学意义(P<0.05),重症肺炎组血清PCT水平显著高于普通肺炎组,比较差异有统计学意义(P<0.05);死亡亚组血清PCT及血乳酸水平均显著高于存活亚组,比较差异均有统计学意义(P<0.05);血清PCT与血乳酸水平呈正相关(r=0.81,P<0.05),并且PCT、血乳酸水平是重症肺炎患儿死亡的独立危险因素。结论:小儿肺炎血清PCT和血乳酸水平明显升高;并且血清PCT和血乳酸水平与病情严重程度、预后有一定关系。 Objective:To investigate the expression level of PCT and Lactic acid in pediatric pneumonia and analyze its relation with the prognosis of severe pneumonia in children.Methods:We selected 159 cases of pediatric pneumonia in our hospital.According to whether it was severe pneumonia, it was divided into ordinary pneumonia(108 cases) and severe pneumonia(51 cases).At the same time, 50 healthy children who came to our hospital for medical examination in the same period were selected as control groups. According to whether the children with severe pneumonia died,it is divided into 40 survival sub-group and 11 death sub-group;the age,sex and time of onset of each group of children were recorded, and the serum PCT and blood lactic acid levels of each group were detected when they were admitted to the hospital.The differences between the levels of PCT and blood lactic acid in each group were compared, and the relationship between them and the prognosis of severe pneumonia was analyzed.Results:There was no statistical significance between the normal pneumonia group and the control group(P>0.05), but there was a significant difference in blood lactic acid in the severe pneumonia group compared with the normal pneumonia group and control group(P<0.05), and the serum PCT of severe pneumonia and common pneumonia was significantly higher than that of the control group, and the difference was statistically significant(P<0.05).The serum PCT level of severe pneumonia was significantly higher than that of normal pneumonia, and the comparative differences were statistically significant(P<0.05);serum PCT and blood lactic acid water in the death subgroup were significantly higher than those in the survival subgroup,and the difference was statistically significant(P<0.05);serum PCT was positively correlated with the level of blood lactic acid(r=0.81,P<0.05), and PCT, blood lactic acid level is an independent risk factor for death in children with severe pneumonia.Conclusion:Serum PCT and serum lactic acid levels were significantly increased in children with pneumonia. And serum PCT and blood lactic acid levels have a certain relationship with the condition of severity and prognosis.
