黄斑下出血的临床特征及玻璃体切除联合眼内电凝手术疗效观察

Clinical characteristics of submacular hemorrhage and the effect of vitrectomy combined with intraocular electrocoagulation

目的探讨黄斑下出血的临床特征以及观察玻璃体切除术联合眼内电凝治疗的临床疗效。方法回顾性分析我院临床确诊为黄斑下出血患者12例(12只眼),均行玻璃体切除联合眼内电凝治疗,记录术前、术后最佳矫正视力(BCVA),将其转换为最小分辨角对数(IogMAR)视力记录观察,并观察眼底情况,平均随访时间为(11.0±4.0)个月。结果治疗后6个月,患眼平均logMARBCVA为(1.16±0.52)μm,与治疗前相比,治疗后6个月患眼BCVA明显提高(t=5.99,P=0.000),差异有统计学意义。所有随访患者未发现视网膜下再出血。结论玻璃体切除联合视网膜电凝治疗黄斑下出血可提高患眼视力;术后应根据患者眼底具体情况,制定个体化治疗方案,维持现有视力。

Objective To investigate the clinical features of submacular hemorrhage and the clinical efficacy of vitrectomy combined with intraocular electrocoagulation.Methods A retrospective analysis was performed based on 12 patients in our hospital who were diagnosed with submacular hemorrhage and underwent vitrectomy combined with intraocular electrocoagulation. The best corrected visual acuity (BCVA) was recorded preoperatively and postoperatively, and was conversed to logarithm of the minimum angle of resolution (logMAR) visual acuity for further calculation. The fundus condition was also observed. The average followed-up duration was 11.0±4.0 months.Results The average logMAR BCVA of the eyes with submacular hemorrhage was 1.16±0.52 um at 6 months after the surgery. The BCVA at 6 months after surgery was significantly better than that before the surgery ( t =5.99, P =0.000). No subretinal hemorrhagic recurrence was observed in all patients during the follow-up.Conclusions The treatment of submacular hemorrhage using vitrectomy combined with retinal electrocoagulation can improve visual acuity. Individualized treatment plans should be developed considering the specific fundus conditions after the surgery to maintain the current visual acuity.
