Analysis and Improvement Measures of Adverse Events in Hospital Pharmacy of a Hospital of Class A and Grade Three

目的 分析住院药房不良事件发生的原因,根据主要原因改进管理措施,减少不良事件的发生。方法 回顾性分析2017—2018年医院住院药房不良事件,通过分类、统计和分析,找出产生不良事件的主要原因,针对主要原因进行有效整改,记录并比较两年不良事件发生情况。结果 针对不良事件的主要原因进行有效整改,减少了不良事件的发生,2018年不良事件下降率约为32%。结论 通过不良事件的分析和学习,改进管理措施,能有效降低住院药房不良事件的发生率,保证临床用药安全。

Objective: To analyze the causes of adverse events in inpatient pharmacy,improve management measures according to the main reasons,and reduce the occurrence of adverse events. Method: Retrospective analysis was conducted on adverse events in the hospital pharmacy from 2017 to 2018.The main causes of adverse events were identified through classification,statistics and analysis,and effective rectification was carried out for the main causes.Adverse events in the two years were recorded and compared. Result: The main causes of adverse events were effectively rectified to reduce the occurrence,and the rate of decrease of the adverse events in 2018 was about 32%. Conclusion: Through the analysis and study of adverse events and the improvement of management measures,the incidence of adverse events in hospital pharmacy can be effectively reduced and the clinical medication safety can be guaranteed.
