白细胞介素与新生儿感染性疾病的关系(附65例报告)

RELATIONSHIP BETWEEN SERUM INTERLEUKINS AND NEONATAL INFECTIOUS DISEASES

目的 探讨新生儿感染性疾病时血清白细胞介素 - 2 (IL - 2 )、白细胞介素 - 6 (IL - 6 )和白细胞介素 - 8(IL - 8)的变化及其临床意义。方法 对 6 5例感染性疾病新生儿和 30例非感染性疾病新生儿用放射免疫法测定血中IL - 2、IL - 6和IL - 8的含量 ,并进行对比观察。结果 感染性疾病组血清IL - 2显著低于非感染性疾病组 (P <0 .0 0 1) ,败血症组IL - 2值显著低于肺炎组或脐炎组 (P <0 .0 5)。感染性疾病组血清IL - 6和IL - 8显著高于非感染性疾病组 (P <0 .0 0 1) ,败血症组IL - 6和IL - 8值显著高于肺炎组或脐炎组 (P <0 .0 5)。结论 新生儿患感染性疾病时存在白细胞介素失调 ,其中IL - 2、IL - 6、IL - 8在其炎症反应过程中可能起重要作用 ,提示白细胞介素水平的高低可反映疾病的严重程度 ,并可作为新生儿感染性疾病早期诊断的一项灵敏指标。

Objective To explore the changes and clinical significance of serum levels of interleukin-2(IL-2), interleukin-6(IL-6) and interleukin-8(IL-8) in neonates with infectious diseases. Methods The concentrations of serum IL-2, IL-6 and IL-8 were tested by radioimmunoassay on 65 cases of infected neonates and 30 cases of noninfectious neonates. Contrastive observation was carried out. Results The concentrations of serum IL-2 in the infected group were significantly decreased compared with that in the noninfectious group (P<0.001),and the serum levels of Il-2 in the septicemia group were significantly decreased compared with those in the pneumonia and omphalitis groups (P<0.05). The concentrations of serum IL-6 IL-8 in the infected group were significantly higher than that in the noninfectious group (P<0.001), and the serum levels of IL-6 and IL-8 in the septicemia group were significantly higher than those in the pneumonia and omphalitis groups (P<0.05). Conclusion When the neonates are infected, there exists irregulation of serum interleukins, and IL-2, IL-6 and IL-8 may play an important role in the process of inflammatory reaction. It is suggested the serum levels of interleukins may reflect the severity of illness. Thus the determination of serum levels of IL-2, IL-6 and IL-8 can be used as a sensitive index for early diagnosis in neonates with infectious diseases.
