摘要
目的 探讨小儿呼气末二氧化碳浓度 (FetCO2 )与动脉血二氧化碳分压 (PaCO2 )的关系以及不同年龄组间是否存在差异。方法 用德国Siemens 930型二氧化碳分析仪和丹麦BME 33型血气分析仪 ,同步测量 119例全麻患儿的FetCO2 和PaCO2 。按照年龄将病人分成甲组 (0~ 1岁 )、乙组 (1~ 4岁 )和丙组 (4~ 9岁 ) ,分别比较其相关关系。结果 全年龄组 119例FetCO2 与PaCO2 呈正相关 ,r =0 .6 14 2 (P <0 .0 5 )。甲组 (30例 )二者虽呈正相关 ,但不够显著 ,r =0 .4 6 2 9(P >0 .0 5 ) ;乙组 (48例 )和丙组 (41例 )二者呈显著正相关 ,r分别为 0 .74 36和 0 .796 8(P均 <0 .0 1)。结论 FetCO2 与PaCO2 有较好的相关关系 ;患儿年龄可影响FetCO2 的准确性 ,在使用中需结合临床 。
Objective To evaluate the correlation between FetCO 2 and PaCO 2 of children and to study the differences between different age groups.Methods The parameters of FetCO 2 of 119 patients undergoing general anesthesia were simultaneously measured by Siemens 930 analyzer and BME 33 blood gas analyzer respectively.The patients were divided into 0~1 years old group (group A,n=30),1~4 years old group (group B,n=48) and 4~9 years old group (group C,n=41).Results The result showed that FetCO 2 and PaCO 2 were positively correlative, r =0.6142 ( P < 0.05 ).FetCO 2 and PaCO 2 in group A were positively correlative but no statistical correlation r= 0.4629 ( P > 0.05 ).In group B and group C,they were positively correlative r= 0.7436 and 0.7968 respectively ( P <0.01).Conclusions The correlation between FetCO 2 and PaCO 2 exists.Age factor affects the accuracy of FetCO 2.FetCO 2 level can represent real PaCO 2 level for the children of older than 1 year.FetCO 2 almost stands for the PaCO 2 accuracy but no good correlation for those of younger than 1 year.We should combine that with the clinic feature to assess ventilatory situation.
