摘要
目的:探讨超声乳化手术中眼压异常增高的机制。方法:总结11例术中发生眼压异常增高的病例观察及处理结果,分析其发生的原因及其机制。结果:①5例属因后囊有小破孔,灌注液迷流所致高眼压。②6例属CCC直径过小,在作水分离时发生。结论:①当后囊有小破孔,在玻璃体皮质成形性较好者有可能发生灌注液迷流,引起高眼压,须减低灌注压或改为囊外手术。②CCC直径不宜过小,水分离时不宜一次大量地推注液体。
Objective: To study the mechanism of high intraocular pressure during phacoemulsification. Methods: 11 cases of high intraocular pressure during phacoemulsification were collected. We analyzed the etiology and clinical characteristic. Results: (1) 5 cases of high intraocular pressure were caused by aberrant irrigation through a small hole on posterior capsule; (2) 6 cases happened for too small CCC while hydrodissection was performed. Conclusion: (1)With small hole on posterior capsule and good plasticity of anterior vitreous, high intraocular pressure may occur because of the aberrant irrigation. Lowing the pressure of irrigation or ECCE is the alternative;. (2)Too small diameter of CCC is not suggested and too much liquid should not be given once when disconnecting by water.
