河南食管癌高发区食管和贲门癌组织中粘蛋白1的表达 被引量:5

Expression of MUC1 in esophageal and gastric cardiac carcinoma tissues from high-incidence area for esophageal cancer in Henan

摘要 目的 :探讨粘蛋白 1(MUC1)表达与河南食管癌高发区食管鳞状细胞癌 (SCC)、贲门腺癌 (GCA)细胞分化程度与淋巴结转移间的关系。方法 :免疫组化ABC法检测MUC1在 5 0例SCC和 2 1例GCA及配对淋巴结组织中的表达 ,分别以活检正常的 10例食管粘膜组织及 8例贲门粘膜组织为对照。结果 :MUC1免疫阳性染色呈棕黄色或深棕黄色。 4 8例SCC(48/5 0 )和 2 1例GCA(2 1/2 1)原发病灶中均有不同程度的MUC1表达 ,6例正常食管粘膜 (6 /10 )和 4例正常贲门粘膜 (4/8)中观察到MUC1弱免疫阳性反应。正常细胞中 ,MUC1主要表达于细胞膜近管腔或腺腔侧 ,但在癌细胞中则胞膜和胞浆中均有表达。SCC与正常食管粘膜、GCA与正常贲门粘膜MUC1免疫阳性反应强度间差异有统计学意义 (分别为P =0 .0 0 5 0 6 ,P =0 .0 0 2 95 ) ,SCC和GCA原发病灶与转移淋巴结间MUC1的一致性表达差异均有统计学意义 (分别为U =4 .4 15 ,P <0 .0 0 0 1,U =2 .2 74 ,P <0 .0 5 ) ,一致性MUC1变化的发生率在SCC为 6 0 % (30 /5 0 ) ,GCA为 85 .71% (18/2 1) ;其中一致性阳性的发生率在SCC为 5 6 % ,GCA为10 0 %。SCC组织分化的恶性程度与MUC1免疫反应的强度呈负相关 (r =- 0 .35 6 6 ,P <0 .0 5 ) ,GCA出现类似趋势 ,但无统计学意义 (P >0 .0 5 )。MUC1免疫反? Aim: To determine the expression of MUC1 in esophageal and cardiac carcinoma and to correlate the alterations of MUC1 expression with cancer cell differentiation and lymph node metastasis.Methods: The level of MUC1 in normal esophageal and cardia mucosa as well as in esophageal squamous cell carcinoma (SCC, n =50), and gastric cardia adenocarcinoma (GCA, n =21) and their matched lymph nodes were determined by immunohistochemical method (ABC). The samples were from a well known high esophageal cancer incidence area in northern China (Linzhou) and nearby county Huixian in Henan Province. Results: High levels of MUC1 expression were observed in the most of the cancer tissues (48/50 in SCC and 21/21in GCA), while weak immunostaining was found in part of normal esophageal (60%) and cardia (50%) mucosa. The levels of MUC1 expression showed significant difference between SCC and normal esophageal mucosa ( P =0.005 06),and GCA and normal cardiac mucosa ( P =0.002 95). The coincidence immunoreaction rate of MUC1 was 60% (30/50) in SCC, and 85.71% (18/21) in GCA; while the coincidence positive immunostaining rate was 56% in SCC, and 100% in GCA. Expression of MUC1 between primary tumor and the matched metastatic lymph node has significant coherence in both SCC and GCA ( P <0.000 1; P <0.05, respectively). Expression of MUC1 showed an inverse correlation with SCC differentiation ( r =-0.356 6, P =0.011). The same tendency was also found in GCA tissues but has no significant difference( r = 0.260 6, P >0.05). The levels of MUC1 expression have linear tendency with lymph nodes metastasis in both SCC and GCA( χ 2=13.39, P =0.000 3). The specificity and sensitivity of using the positive immunoreaction for MUC1 in lymph node to evaluate whether metastasis happened were 100% and 87.04%, respectively. Conclusion: The alteration of MUC1 expression is an important mark of the lymph node metastasis of SCC and GCA, and may play an important role in the carcinogenesis of SCC and GCA. The expression of MUC1 may be effected by the grade of differentiation.
