猫抓病淋巴结细针穿刺细胞学分析

Fine needle aspiration cytological analysis in lymph nodes of cat-scratch disease

目的 :探讨猫抓病 (cat scratchdisease,CSD)淋巴结细针穿刺细胞学及免疫组化特点。方法 :对 1 7例CDS患者的淋巴结细针穿刺资料进行细胞学、组织学及免疫组化分析。结果 :1 7例CSD患者均有与猫密切接触史 ,并伴局部淋巴结肿大 ,其HE染色细胞学特点为 :可见呈小簇状或栅状分布的上皮样组织细胞 ,较多中性粒细胞及无定形的坏死碎片 ,偶见吞噬核碎片的巨噬细胞 ,淋巴细胞增生 ,大小不一 ,可见多种转化阶段的淋巴样细胞。免疫表型 :CEA、EMA均 (- ) ,CD45(+) ,CD2 0、CD45RO(+/ - ) ,CD68( ) ,Mac387( )。结论 :CSD是一种病程自限的细菌感染性疾病 ,其临床病史、淋巴结细针穿刺细胞学检查、免疫组化。

Purpose To investigate the cause of cat scratch disease(CSD) and its fine neddle aspiration cytology(FNAC) features and immunohistochemical expressions. Methods Seventeen cases FNAC of lymph nodes of CSD were analyzed by cytological and immunohischemical techniques. Results All of the patients had contact history with cat. The local lymph nodes were punctured swelling. It was obviously that epithelioid histocytes arranged in areatused or palisaded. The lymphocytes of many kinds of conversion phase and neutrophils and sphacelus could be seen. The macrophages that phagocytized the fragment of nucleus occassional could be found. CEA(-), EMA(-),CD45(+),CD20(+/-),CD45RO(+/-),CD68(),Mac387() were observed in immunohischemistry. Conclusion CSD is a kind of self limited duration and infective disease. Its clinical and cytological features and immunohischemical expressions and specific intracutaneous test have important meaning in its diagnosis and differential diagnosis.
