摘要
本实验根据门脉高压巨脾病理分级标准,挑选纤维化巨脾Ⅰ~Ⅲ级各一例。于H-500电镜下观察其超微结构改变,主要观察胶原纤维的增生分布特点,脾脏窦壁上内皮、基膜和网状细胞的改变,具有吞噬功能的各种细胞吞噬现象的改变和某些亚细胞结构的改变。发现:Ⅰ级巨脾以淤血为主,纤维性增生较轻,但仍有反映脾脏良好吞噬滤过功能的组织形态:Ⅱ级巨脾纤维化较为明显,胶原向脾索窦壁延伸,致窦壁三层结构显著改变,吞噬细胞的某些亚细胞结构异常:Ⅲ级巨脾显示严重的纤维化,胶原广泛分布,某些正常的脾脏结构消失,视野内难以发现巨噬细胞和浆细胞。
According authors classification of splenomegaly in portal hypertension, this study selected three cases of splenomegaly with different fibrosis, examining its ultrastructure under a Hitachi H-500 electron microscope, the main observation were. ( 1) The distrubuting features of collagen proliferation. (2) Changes of endothelium, basement membrane and adventitia on the splenic sinus wall. ( 3 ) Phagocytosis phenomenon of different cells. ( 4 ) Some sub- cellular structure. The study found. ( 1 ) In class Ⅰ, the splenomegalies were mainly congestive, light fibroproliferation, there are some features showing a good function of phagocytosis and clearance.(2)In class I ,the fibro-sis was marked, collagen branch into sinus wall and extend into cord, which changes the three layer of sinus wall and subcellular structure of phagocytes. ( 3 ) There are severe fibrosis of splenomegaly, collagen distributed extensively, some normal splenic structure disappeared, few macrophages and plasma cells can be found.
