摘要
目的:研究原发性高血压(EH)不同阶段外周静脉血流动力学参数变化。方法:应用应变容积描记及光容积描记法,对650例受检者做下肢外周静脉排血量(VO)、外周静脉容量(VC)、外周静脉瓣膜关闭功能(VRF)测定。其中EH3级患者共50例,按分层随机对照原则,与同期EH 1~2级、正常血压对照组各50例作对比分析。结果:EH组右下肢及左下肢VO下降18%~38%,右下肢及左下肢VC下降16%~28%,反映右下肢及左下肢VRF的静脉倒流时间缩短7%~23%。结论:EH易引起VO及VC下降、VRF异常。这些静脉系统病变的变化始于EH 1~2级阶段,至3级阶段所有3项静脉血流动力学参数均转为异常,表明EH存在严重外周静脉血流动力学紊乱,静脉病变与EH病变严重程度有密切关系。
Objective:To study the changes of hemodynamics in peripheral veins of essential hypertension. Methods: Venous outflow (VO), venous capacitance (VC) and venous refilling time (VRF) were measured by strain gauge plethysmography (SPG) and photoplethysmography (PPG). The parameters mentioned above were compared among three groups which consisted of normotensive, grade 1~2 and grade 3 hypertensive groups with 50 cases in each one. Results: VO,VC and VRF decreased ranging from 18%~38%, 16%~28% and 7%~23% respectively, which developed not only in right but also in left lower extremities (0. 01<P <0. 05 or P <0. 01). Conclusion :Essential hypertension tends to cause hemodynamic disturbances in peripheral veins. The reduction of VO, VC and VRF were begun in the grade 1-2 group and significanfly declined in the grade 3 hypertensives. These hemodynamic changes show that there in the close relationship between the abnormality of peripheral veins and the severity of essential hypertension.
