APPLICATION OF 24h - AEEG IN THE JUDICIAL APPRAISE OF CRIMINAL WITH DOUBTFUL EPILEPSY

目的:探讨动态脑电图(AEEG)在可疑癫痫违法司法精神医学鉴定中的应用价值。方法:对举荐的46例可疑癫痫犯罪人的24小时动态脑电图进行回顾分析。结果:46例中,痫样放电13例(28.3%),非特异性异常8例(17.4%),总异常率为45.7%;痫样放电出现清醒期3例(23.1%),睡眠期出现10例(76.9%),两期有显著性差异(P<0.01);部位多见于额、颞区局限性异常。结论:AEEG因监测时间长,对司法精神医学鉴定中可疑癫痫犯罪的鉴别诊断可提供重要的依据,但不作为责任能力的判定依据。

Objective To study the applied value of 24h - AEEG in appraise of judicial psychologic medicine of criminal with doubtful epilepsy. Methods A retrospective analysis of 24h - AEEG was done in 46 criminal with doubtful epilepsy recommend by judicial organization. Results In 46 criminal with doubtful epilepsy examined with 24h - AEEG, there were epileptiform discharges in 13 cases (28.3%), nonspecific abnormal AEEG was noted in 8 criminal with doubtful epilepsy. The abnormal rate of AEEG was 45.7 % . The time of epileptiform discharge was mainly in sleeping stage (76.9%), and that was 23.1% in the waking stage. There was significant difference between two stage (P < 0.01) . The local abnormal discharge was mostly seen on the frontal and temporal lobes. Conclusions AEEG can provide an important basis of differential diagnosis in appraise of judicial psychologic medicine by virtue of long-monitoring, however, it is not the basis of judging responsi- bility.
