摘要
目的 :了解呼吸机相关肺炎 (VAP)病原菌来源。方法 :3 5例患者采取胃液、口咽部、下呼吸道、呼吸机进出管道、加温湿化器、医务人员手部吸痰后标本 ,对比其菌群组成。结果 :各部位均有病原菌检出 ,医务人员手部吸痰后病原菌检出率较高。检出的细菌较多的有铜绿假单胞菌、克雷白杆菌、大肠埃希菌、金黄色葡萄球菌、不动杆菌。各部位检出病原体中 ,真菌均占相当大的比例。医护人员的手采集标本培养结果与痰培养结果符合率较高为 60 .7%。结论 :人工气道机械通气患者胃液、口咽部、未严格消毒的呼吸机回路、医护人员的手均有致病菌检出 。
Objective:To study the pathogeny source of ventilator-associated pneumonia(VAP).Methods:Thirty-five patients accepted intubation and mechanical ventilation were involved in our study.Microbiologic cultures of specimens were performed which are taken from the sputum,gastric juice,oropharynx,ventilator circuits,humidifiers and hands of the staff in the ICU.Results:The pathogeny can be found in all the sites where the specimens were taken.There were more pathogeny in the hand than in other sites.the main pathogeny were pseudomonas aeruginosa,klebsiella,escherichia coli,staphylococcus aureaus and acinetobacter.The fungus (mainly candida albicans)were easily found in all the sites.The correspondence rate of the culture results between the sputum and the hands was 60.7%.Conclusion:There was pathogeny in all the place where the specimens were taken,which may be cause VAP.
