增殖细胞核抗原检测在非霍奇金淋巴瘤中的临床意义

目的 研究非霍奇金淋巴瘤 (NHL)中增殖细胞核抗原 (PCNA)的表达水平 ,探讨其在NHL中的临床意义。方法 应用免疫组化法检测 5 3例NHL患者和 10例淋巴结反应性增生患者 (对照组 )石蜡切片中PCNA的表达。结果 10例对照组中PCNA表达以淋巴滤泡中心为主 ,均为低度阳性。 5 3例NHL患者PCNA表达全部为阳性 ,但强度明显不同。低度恶性组PCNA表达均为 (+ )～ (+ + ) ,无 (+ + + )者 ,中、高度恶性组均为 (+ + )～ (+ + + ) ,且 (+ + + )占 42 .2 %(19/4 5例 )。Kaplan Meier生存曲线显示 ,PCNA(+ + )组生存率明显高于PCNA(+ + + )组 (P <0 .0 1)。

Objective To assess the clinical significance of the expression level of proliferating cell nuclear antigen (PCNA) in non Hodgkin's lymphoma (NHL).Methods Immunohistochemical assay was used for PCNA expression. Results PCNA was positive in all cases with NHL and the control group (reactive lymph node hyperplasia).But the intensity was different, in low malignant grade group, PCNA was(+)～(++),in intermediate grade and high grade groups,PCNAs were(++)～(+++),in which PCNA(+++) accounted for 42.2%(19/45 cases),in control group it was(+).There was a strong correlation between the level of PCNA expression and prognosis( P <0.01).The two year survival rate was 63.82% for the patients with PCNA(++),7.06% for the patients with PCNA(+++) ( P <0.01).Conclusion The positive degree of PCNA might be an important index for evaluating malignant grade and prognosis of NHL.
