多电极射频联合肝动脉栓塞化疗治疗肝癌的应用研究

摘要 目的 探讨多电极射频联合肝动脉栓塞化疗对肝癌治疗的方法及效果。方法 经皮穿刺组织病理学证实的 16例肝癌 31个结节 ,多电极射频治疗后 1~ 2周 ,行肝动脉栓塞化疗。结果 经选择性肝动脉造影证实 ,12个小结节完全坏死 ,占小结节的 85 7% (12 / 14 ) ,17个临近肝脏边缘的较大结节及 2个特殊部位的小结节有肿瘤残留 ,占所有结节的 6 1 3% (19/ 31)。其中 3个较大的结节联合栓塞化疗后 3个月仍见肿瘤生长。 16例患者半年荷瘤生存率 75 %(12 / 16 )。所有患者治疗后临床症状明显改善。结论 先射频后栓塞的方法可以发现并治疗肿瘤残留 ,提高疗效。 Objective To assess the method and the effect of combining percutaneous radiofrequency ablation with thanscatheter arterial chemoembolization to the hepatoma. Methods 31 nodules in 16 patients,who were biopsy proved hepatoma,were treated by the thanscatheter arterial chemoembolization 1 week or 2 after percutaneous radiofrequency ablation. Results Complete response,partial response(residual hepatoma)were found on selective hepatic angiography in 12 of 14 small nodules(85.7%),at the edge of the tumor near the liver surface in 17 big nodules and at the special site in 2 small ones(61.3%),respectively.Local tumor growth was found 3 months after the combining therapy.The disease free survival was 75%(12/16)more than half a year.The manifestation was obviously improved in all the patients. Conclusion The way of combining percutaneous radiofrequency ablation with thanscatheter arterial chemoembolization can find and treat the residual nodules.Therefore,it can increase the therapeutic effects of the disease.
