摘要 目的 观察12 5碘粒子永久植入联合巨能射频消融治疗前列腺癌的疗效 .方法 在直肠B超引导下 ,经会阴模板穿刺将12 5碘粒子植入到靶组织中 ,再行巨能射频消融 ,治疗前列腺癌病人 12例 .结果 随访 2~ 12个月 ,10例PSA术后降至正常 ,直肠B超发现前列腺缩小 ,部分钙化改变 ,2例术后PSA下降又反弹升高 ,需加用激素类药物治疗 .12例手术过程顺利 ,无出现严重并发症 .结论 12 5碘粒子植入联合巨能射频消融治疗前列腺癌疗效满意 ,是一种微创 ,安全 ,并发症少的方法 。 Objective To assess the effectiveness of 125I brachytherapy with interstitial radiofrequency in prostate cancer. Methods From 2001/7 to 2002/6,12 patients with prostate cancer diagnosed by pathologic examination ,were treated with 125I seed implants combined with transperineal interstitial radiofrequency under transrectal ultrasound guidance. Results It was no side effects on liver or kidney function damage. Seven cases had a slight increase in urinary frequency,urgency and nocturia with 1 moths after treatment.IPSS and volume of prostate increased but not significant at 1 monlhs after treatment(p>0.05).One case occurred dysuria at 3 days after therapy when removed the catheter.PSA decreased significantly at 1,3,6,months after therapy compared with preoperation(p<0.05) IPSS and volume of prostate also decreased significantly compared with pretreatment at 3,6 months after therapy. Conclusion 125I seed permanent implanting (brachytherapy) combined with transperineal interstial radiofrequency for the treatment of prostate cancer was effective and feasible in short period ,the amounts of seeds implanting reduced obviously with the combination of two individual treatment modalities than brachytherapy alone,the long-term effects should be assessed further.
