脐围代替腰围在诊断代谢综合征中的价值

The Value of Navel Circumference instead of Waist Circumference in Diagnosing Metabolic Syndrome

目的探讨脐围与腰围的相关性及使用脐围替代腰围用于诊断代谢综合征(MS)的可行性。方法选取广东药科大学附属第一医院消化科入院治疗的且能平稳站立并配合测量腰围与脐围的MS患者,收集患者的腰围、脐围、血脂、血糖、血压等临床资料,使用Spearman相关分析脐围与腰围的相关性。以腰围诊断MS为金标准,计算以脐围诊断MS的敏感度、特异度、阳性预测值、阴性预测值、约登指数。结果脐围与腰围呈高度正相关(r=0.895,P<0. 001),使用脐围诊断MS的敏感度为92.79%(103/111);特异度为97.49%(272/279);阳性预测值为93.64%(103/110);阴性预测值为97.14%(272/280);约登指数为90.28%。结论脐围的测量简便易行,用于诊断MS的准确性较高,可替代腰围诊断MS。

Objective To assess correlation between waist circumference and navel circumference and investigate the feasibility of navel circumference instead of waist circumference in diagnosis of metabolic syndrome.Methods Patients with metabolic syndrome in our hospital were able to stand steadily and cooperate with the measurement of waist circumference and navel circumference. The clinical data such as waist circumference,navel circumference,blood lipids,blood sugar and blood pressure were collected.The Spearman correlation test was used to assess the correlation between waist circumference and navel circumference.The metabolic syndrome was diagnosed with waist circumference as the standard and the sensitivity,specificity,positive predictive value,negative predictive value,and Yoden index of the metabolic syndrome were calculated. Results It had a high positive correlation between navel circumference and waist circumference( γ = 0.995,P<0.001).The sensitivity of navel circumference in diagnosis of metabolic syndrome was 92. 79%( 103/111),the specificity was 97. 49%( 272/279),the positive prediction value was 93.64%( 103/110),the negative prediction value was 97.14%( 272/280) and the Yoden index was 90.28%.Conclusions The navel circumference measurement is simple and easy,and it has high accuracy for diagnosing metabolic syndrome.It can replace waist circumference to diagnose metabolic syndrome.
