严重腹腔感染时胃粘膜细胞胞液糖皮质激素受体变化对胃粘膜屏障功能的影响

摘要 目的 了解严重腹腔感染时胃粘膜细胞胞液中糖皮质激素受体变化对胃粘膜屏障功能的影响。方法 将 12 0只雄性Wistar大鼠 ,随机分成实验组 (n =60 )和对照组 (n =60 ) ,实验组利用大鼠盲肠结扎穿孔造成严重腹腔感染动物模型 ,分别采用电生理法和放射免疫分析法测定了穿孔前、穿孔后 3、6、12、2 4、48h胃粘膜电位差变化和胃粘膜组织皮质醇含量变化 ,应用放射配体分析法对各时相胃粘膜细胞胞液糖皮质激素受体进行了测定。结果 盲肠穿孔后 3h胃粘膜组织皮质醇含量明显升高 ,12h达高峰 ,而胃粘膜细胞胞浆内糖皮质激素受体的最大结合容量显著下降 ,平衡解离常数 (KD值 )明显增大 ,胃粘膜电位穿孔后显著下降。结论 糖皮质激素受体水平降低是严重腹腔感染后急性胃粘膜屏障功能受损的重要原因之一。 Objective To investigate the effect of glucocorticoid receptor(GCR) in cytoplasm of gastric mucosal cells on gastric mucosal barrier following severe intraperitoneal infection. Methods A total of 120 male Wistar rats weighing between 190 g and 250 g were randomly divided into 2 groups of 60 rats each (infected group and control group). Intraperitoneal infection rat model was established by cecal ligation and puncture(CLP). Then the dynamic changes of glucocorticoid receptor in cytoplasm of gastric mucosal cells and gastric mucosal potential difference(PD) were determined in rats suffering from severe intraperitoneal infection at pre perforation, 3, 6, 12, 24 and 48 h after CLP. Results After severe abdominal infection, the cortisol level in gastric mucosa increased significantly but the number and affinity of GCR in cytoplasm of gastric mucosal cells and gastric mucosal potential difference decreased markedly. Conclusion GCR plays an important role in the acute gastric mucosal damage after severe intraperitoneal infection. The reduction of GCR level may result in dysfunction of gastric mucosal barrier.
