输卵管妊娠药物治疗过程中手术时机选择

Choice of operative occasion during the treatment of tubal pregnancy by pharmacotherphy

目的 探讨输卵管妊娠药物治疗过程中如何选择手术时机。方法 对 42例未破裂型输卵管妊娠行药物治疗 ,动态观察β hCG值及包块大小的变化。 结果 42例中 2 2例药物治愈 ,2 0例因 β hCG急剧升高或者维持高值 ,包块增大或长时间不缩小而以手术治愈。结论 药物治疗输卵管妊娠过程中应根据 β hCG值和包块大小的变化来选择手术时机。当 β hCG值 >2 0 0 0IU/L ,包块直径 >3cm时药物治疗极有可能失败 ,应及时手术。

Objective To evaluate how to choose the operative occasion during the treatment of tubal pregnancy by pharmacotherapeutic method.Methods Fourty-two tubal pregnant patients without rupture were treated by pharmacotherapeutic method.The changes of mass size and β-hCG were observed dyna mically.Results Twenty-two of them were successfully treated,20 of them needed to be treated by surgical procedure because of rapid-increasing or remaining high level of β-hCG,the increasing or non-reduction of mass-area for long time.Conclusion Operative occasion during the treatment of tubal pregnancy by pharmacotherapeutic method should be chosen according to the changes of β-hCG and mass size.When the value of β-hCG is higher than 2000IU/L and the diameter of the mass is larger than 3cm,the drug treatment may be failure,and operation should be choosed in time.
