Ilizarov胫骨外翻截骨术联合补肾活血颗粒治疗膝骨关节炎30例

Osteotomy Correction of Ilizarov Tibial Valgus Combined with Bushen Huoxue Granules Treatment for Knee Osteoarthritis in 30 Cases

目的观察Ilizarov胫骨外翻截骨术联合补肾活血颗粒治疗膝骨关节炎的临床疗效。方法选择60例肾虚血瘀型轻中度且伴有不同程度的内侧关节间隙变窄的膝骨关节炎患者,随机分为治疗组和观察组各30例,治疗组采用Ilizarov胫骨外翻截骨术治疗,观察组采用Ilizarov胫骨外翻截骨术并术后口服补肾活血颗粒治疗6周,比较2组治疗前后WOMAC评分及关节滑液NO、IGF-1、TNF-ɑ含量。结果 2组治疗后关节滑液NO、IGF-1、TNF-ɑ含量及WOMAC评分与治疗前比较均明显改善(P<0.05),而治疗后比较观察组改善更明显(P<0.05)。结论 Ilizarov胫骨外翻截骨术治疗膝关节骨关节炎疗效显著,而其术后配合补肾活血颗粒治疗肾虚血瘀型膝骨关节炎疗效更好。

Objective:To observe the clinical effect of Ilizarov’s tibial eversion osteotomy in the treatment of knee osteoarthritis,and to understand the promotion effect of oral Bushen Huoxue granules.Methods:Select sixty patients with mild-to-moderate kidney and blood stasis type and different degree of the patients with knee osteoarthritis of the medial joint space narrow.The patients were randomly divided into treatment group and observation group,with 30 cases in each group.The treatment group was treated with Ilizarov tibia valgus osteotomy,while the observation group uses the Ilizarov tibial valgus osteotomy combined with postoperative oral kidney invigorate for 6 weeks.The NO,IGF-1,TNF-αcontent of joint synovial fluid and WOMAC score of two group were compared.Results:The NO,IGF-1,TNF-αcontents and WOMAC scores of the two groups were significantly improved after treatment compared with that before treatment,while the improvement was more obvious in the observation group after treatment(P<0.05).Conclusion:Ilizarov’s tibial eversion osteotomy is effective in treating knee osteoarthritis,and its postoperative treatment of knee osteoarthritis with kidney-deficiency and blood-stasis type is better with Bushen Huoxue granules.
