围绝经期功能失调性子宫出血治疗中应用孕激素的疗效评价

Perimenopausal dysfunctional effect of progesterone in the treatment of uterine bleeding

目的评价围绝经期功能失调性子宫出血治疗中应用孕激素的疗效。方法将我院2011年3月~2013年10月收治并确诊为围绝经期功能失调性子宫出血的90例患者随机分为观察组和对照组,各45例。所有患者行刮宫术后,对照组患者应用达英-35治疗,对观察组患者应用孕激素安宫黄体酮治疗。观察两组患者的疗效,同时观察两组患者治疗期间不规则出血例数、第3周期月经量恢复正常例数、恶心呕吐例数、停药半年复发率。结果两组疗效比较中,观察组的有效率为71.1%明显低于对照组的93.3%,差异有统计学意义(P<0.05);对照组的治疗期间不规则出血例数、第3周期月经量恢复正常例数明显多于对照组,恶心、呕吐等并发症的发生率明显低于观察组,差异均有统计学意义(P<0.05);两组患者停药半年复发率的比较,差异无统计学意义(P>0.05)。结论孕激素安宫黄体酮治疗围绝经期功能失调性子宫出血患者,对降低患者子宫内膜厚度,提升患者血红蛋白值具有一定的效果,但是达英-35对围绝经期功能失调性子宫出血具有副作用小、止血效果好的优势,比安宫黄体酮的疗效更加显著。

Objective To evaluate the effect of perimenopausal of dysfunctional uterine bleeding applying to progesterone treatment. Methods In our hospital from March 2011 to 2013 patients were treated and diagnosed as perimenopausal dysfunctional uterine bleeding, 90 patients were randomly divided into observation group and control group 45 cases, and all patients underwent curettage of control patients mifepristone treatment, patients in the observation group progestin medroxyprogesterone acetate treatment. The efficacy of the two groups were also observed two groups of patients before and after treatment of endometrial thickness, changes in hemoglobin. Results Comparison of efficacy results in the observation group was significantly lower than the 71.1%effective rate of 93.3%in the control group, the difference had a statistically meaning (P<0.05); endometrial thickness in both groups of patients after treatment than before treatment significant reduction in endometrial thickness after comparing two groups of patients in the observation group was significantly higher, the difference had a statistically meaning(P<0.05); two groups in hemoglobin values after treatment than before treatment significantly increased large groups of patients after treatment in comparison hemoglobin values, the observation group was significantly lower than the control group out, the difference had a statistically meaning (P<0.05). Conclusion Perimenopause treatment of dysfunctional uterine bleeding applying to progesterone can reduce the thickness of the endometrium of patients and improve patient hemoglobin value has some effect.And the efficacy of mifepristone is more significant.
