摘要
为探讨三维超声技术检测胎儿体表形态和排除胎儿畸形的临床应用价值 ,采用静态瞬时超声三维表面成像技术 ,对 14 5例妊娠 10~ 4 2周的胎儿进行体表结构和形态的重建。结果显示 :在有一定羊水量包围的胎儿体表区域 ,三维超声表面成像技术可检测胎儿体表的任何感兴趣区域 (ROI)部位的宏观形状及细微特征 ,如轮廓、边界、平滑度等。由此 ,可重建胎儿面部不同角度的镜像形态 ,包括面部各个器官的发育形态 ,并可重建胎儿肢体和手指、足趾的形态和数量 ,以及呈螺旋状的脐带。 14 5例中有 132例成功地建立了胎儿体表不同部位的三维图像 ,成像率达 91.0 3%。一次性超声检查诊断胎儿体表有无畸形的正确率达 85 .6 1%。结果提示 :三维超声技术比二维超声更为详细和全面的提供胎儿宫内生长发育的形态信息和数据 。
To study the practical value of the three dimensional (3D) ultrasound detecting fetal surface structure and malformation,3D fetal surface structure images were reconstructed in 145 cases from 10 to 42 gestational weeks to exclude fetal surface malformation and defects by means of 3D sonographic static surface mode technique. The results showed that with 3D quasi time static surface mode, fetal surface 3D images were reconstructed successfully in 132 of 145 cases (91 03 %). Most surface areas of the fetus have been shown clearly in the prerequisite of the fetal region of interest (ROI) surrounded by enough amniotic fluid. Furthermore, this technique could detect the fetal surface subtle characters such as general face image, figure, margin, smoothness and similarity between the fetus and his/her parents. The correct diagnostic rate to exclude fetal malformation was 85 61 % in one time examination in our study. The study indicated that 3D ultrasound supplied more abundant, detailed and comprehensive information of fetal surface image, configuration and growth situation than two dimensional ultrasound. 3D ultrasound technique provides more useful and reliable data and parameters, and high quality 3D imaging is one of research trend on non invasive prenatal diagnosis.
