颅中窝径路解剖定位的临床意义

摘要 目的 为颅中窝入路手术提供相应的解剖学依据 .方法 对 2 0具甲醛固定的成人尸颅进行测量 ,确定翼点、颧弓点至颅中窝各结构及面神经管裂孔至弓状隆起、内耳门、棘孔的夹角和距离 .结果 翼点至圆孔、卵圆孔、棘孔、三叉神经压迹、面神经管裂孔、弓状隆起、内耳门的连线长度和各连线与正中矢状面成角分别为 (5 8.0± 3.7) mm(71.5± 6 .4 )°,(5 9.9± 3.9) mm (5 6 .2± 6 .4 )°,(5 9.2± 3.8) mm (49.2±6 . 8)°,(6 3.0± 3.8) mm (5 2 .8± 6 .7)°,(5 9.6± 3.6 ) mm(41.0± 5 .4 )°,(6 2 .9± 4 .7) mm(2 7.3± 5 .6 )°,(6 9.5± 4 .1)mm (40 .3± 5 .1)°.颧弓点至圆孔、卵圆孔、棘孔、三叉神经压迹、面神经管裂孔、弓状隆起、内耳门的连线长度和各连线与正中矢状面成角分别为 (5 2 .4± 3.0 ) mm (90 .2± 5 .6 )°,(48.5± 3.9) mm (74 .4± 6 .1)°,(44 .4± 2 .6 ) mm (6 5 .9±5 .8)°,(5 3.7± 3.4 ) mm (6 6 .3± 5 .9)°,(47.0± 2 .6 ) mm(5 4 .6± 7.2 )°,(5 4 .1± 3.3) mm(40 .7± 5 .8)°,(6 0 .1± 2 .8)mm (5 3.4± 5 .9)°.面神经管裂孔至弓状隆起、内耳门、棘孔的连线长度和各连线与正中矢状面成角分别为 (17.5± 2 .8)mm(337.2± 9.2 )°,(14 .5± 1.2 ) mm(44 .3± AIM To privde anatomical basis for surgical operation in media cranial fossa. METHODS The length of lines linking pterion and zygion to the structures in fossa cranii media and the angles between these lines and median vector were measured in 20 adult head cadavers fixed with formalin. The same measurements of tarin foramen to arcuate eminence, internal acoustic pore and foramen spinosum were also taken. RESULTS The distances and angles from pterion to foramen rotundum, foramen ovale, foramen spinosum, trigeminal impression, tarin foramen, arcuate eminence, internal acoustic pore were (58.0±3.7) mm (71.5±6.4)°, (59.9±3.9) mm (56.2±6.4)°, (59.2±3.8) mm (49.2±6.8)°, (63.0±3.8) mm (52.8±6.7)°, (59.6±3.6) mm (41.0±5.4)°, (62.9±4.7) mm (27.3±5.6)°, (69.5±4.1) mm (40.3±5.1)°, respectively. The same items mentioned above form zygion to foramen rotundum, foramen ovale, foramen spinosum, trigeminal impression, tarin foramen, arcuate eminence, internal acoustic pore were (52.4±3.0) mm (90.2±5.6)°, (48.5±3.9) mm (74.4±6.1)°, (44.4±2.6) mm (65.9±5.8)°, (53.7±3.4) mm (66.3±5.9)°, (47.0±2.6) mm (54.6±7.2)°, (54.1±3.3) mm (40.7±5.8)°, (60.1±2.8) mm (53.4±5.9)°, respectively. The distances and angles from tarin foramen to arcuate eminence, internal acoustic pore and foramen spinosum were (17.5±2.8) mm (337.2±9.2)°, (14.5±1.2) mm (44.3±12.5)°, (10.7± 3.5 ) mm (156.8±40.3)°, respectively. CONCLUSION The measurements provide some useful references for defining the position in operation.
